摘要
目的比较活动平台膝关节与固定平台膝关节单髁置换术(UKA)后下肢力线的矫正效果。方法回顾分析2013年10月至2018年10月在河南省直第三人民医院骨科行UKA的229例患者(242膝)的病历资料。根据假体的不同分为活动平台组(174例、182膝)和固定平台组(55例、60膝)。活动平台组选用OxfordⅢ单髁假体治疗,固定平台组选用Zimmer M/G单髁假体治疗。比较两组患者手术前后的髋-膝-踝角(HKA)、胫股角(FTA)及胫骨后倾角(PSA)。结果手术前后两组HKA、FTA比较,差异均无统计学意义(均P>0.05);但两组HKA、FTA矫正差值比较,差异均有统计学意义(均P<0.05),且活动平台较固定平台矫正下肢力线角度更大。两组患者UKA术后PSA角较术前均有改变,但手术前后差异均无统计学意义(均P>0.05)。结论活动平台UKA比固定平台UKA矫正下肢冠状面力线的角度更大,对于恢复下肢解剖学力线更具优势。
Objective To compare the alignment correction and outcomes between fixed-bearing and mobile-bearing unicompartmental knee arthroplasty(UKA).Methods The medical records of 229 patients(242 knees)who underwent UKA in the Department of Orthopedics,Henan Third Provincial People's Hospital between October 2013 and October 2018 were retrospectively analyzed.All patients were set into mobile-bearing group(174 cases,182 knees)and fixed-bearing group(55 cases,60 knees)according to the differences of the prosthesis.The mobile-bearing unicompartmental knee prosthesis was Oxford phaseⅢ.The fixed-bearing unicondylar knee prosthesis was Zimmer M/G.The hip-knee-ankle angle(HKA),femorotibial angle(FTA)and posterior slope angle(PSA)were measured before and after surgery.Results There were no significant difference in HKA and FTA between the 2 groups before and after surgery(all P>0.05).There were statistically significant differences in HKA and FTA between the 2 groups before and after surgery(all P<0.05),and the angle of alignment corrected by mobile-bearing was larger than that by fixed-bearing.The PSA angles of UKA patients were changed after surgery,but there was no significant difference in PSA between the 2 groups before and after surgery(all P>0.05).Conclusion The mobile-bearing UKA has a larger angle of correction than the fixed-bearing UKA in the coronal plane,and it is more advantageous for restoring the lower limb alignment.
