摘要
目的探讨单纯性尿道狭窄经等离子电切联合球囊扩张导管治疗对排尿功能的影响。方法选取2017年4月至2018年6月收治的45例男性单纯性尿道狭窄患者,采用数表法随机分为联合组和对照组,联合组应用等离子电切联合球囊扩张术治疗,对照组应用单纯输尿管镜钬激光内切开术治疗,观察两组患者临床指标。结果联合组术前住院时间、术后住院时间、尿管冲洗时间与对照组差异不具有统计学意义(P>0.05);联合组手术时间长于对照组(P<0.05);联合组术中出血量低于对照组(P<0.05);术前,联合组Qmax、残余尿量(post voiding residual volume,PVR)、IPSS评分、QOL评分与对照组差异不具有统计学意义(P>0.05);术后3个月,联合组PVR、IPSS评分低于对照组(P<0.05),联合组Qmax高于对照组(P<0.05),联合组QOL评分与对照组差异不具有统计学意义(P>0.05);术后1个月,联合组并发症发生率低于对照组(P<0.05)。结论等离子电切联合球囊扩张导管术治疗可有效改善男性单纯性尿道狭窄患者排尿功能,安全性优于单纯输尿管镜钬激光内切开术。
Objective To investigate the effect of plasma electrotomy combined with balloon dilation cath eter on urinary function in patients with simple urethral stricture.Methods A total of 45 male patients with ure thral stricture admitted from April 2017 to June 2018 were selected and randomly divided into the control group and the combination group by number table method.The control group was treated with ureteroscopic holmium laser internal resection,and the combination group was treated with plasma electrotomy combined with balloon dilata tion.Clinical indicators of the two groups were observed.Results The preoperative hospitalization time,postopera tive hospitalization time and urinary tube washing time of the combined group showed no statistical significance(P>0.05).The operation time of the combined group was higher than that of the control group(P<0.05).Intraoperative blood loss in the combined group was lower than that in the control group(P<0.05).Before the operation,Qmax,PVR,IPSS and QOL scores of the combined group showed no statistical significance with control group(P>0.05).Three months after the operation,PVR and IPSS scores in the combined group were lower than those in the control group(P<0.05),Qmax in the combined group was higher than that in the control group(P<0.05),The QOL scores between the combined group and the control group showed no statistical significance(P>0.05);One month after the operation,the incidence of complications in the combined group was lower than that in the control group(P<0.05).Conclusion Plasma electrotomy combined with balloon dilatation catheterization can effectively improve the urination function of male patients with simple urethral stricture,and its safety is better than that of ureteroscop ic holmium laser internal resection.
