摘要
目的:探讨超声引导下经皮粗针穿刺活检(CNB)在周围型肺占位病变中的应用价值。方法:回顾性分析2010年6月—2017年9月在常州市第二人民医院超声科行超声引导下经皮粗针穿刺活检术的肺周围型占位病变56例,将穿刺活检结果与临床最终诊断结果进行对比,分析超声引导下周围型肺占位穿刺活检的准确性、取材成功率及并发症。结果:56例周围型肺占位病变穿刺后病理证实恶性病例34例(60.71%);良性病例18例(32.14%);未能诊断病例4例(7.14%)。恶性病例其中3例行手术治疗,术后病理证实;1例穿刺未能诊断病例行手术证实为恶性。56例病例均一次性取材成功(100.0%),总准确性92.85%(52/56)。所有超声引导下经皮粗针穿刺活检术的肺周围型占位病变均未出现严重并发症。结论:超声引导下经皮周围型肺占位病变粗针穿刺活检是一种安全、高效的检查方法,可以作为周围型肺占位病变诊断的可靠手段。
Objective To discuss the application value of ultrasound-guided percutaneous core needle biopsy(CNB)in peripheral pulmonary space-occupying lesions.Methods 56 cases of peripheral pulmonary space-occupying lesions of ultrasound-guided percutaneous core needle biopsy(CNB)from June 2010 to September 2017 in Changzhou Second People’s Hospital were retrospectively analyzed,the biopsy results were compared with the clinical final diagnosis results,and the accuracy,sampling success rate and complication of ultrasound-guided percutaneous core needle biopsy.Results Among 56 cases of peripheral pulmonary space-occupying lesions,34(60.71%)were confirmed to be malignant ones;18(32.14%)were confirmed to be benign ones(32.14%);and 4 cases failed to be diagnosed(7.14%).Among the malignant cases,3 had surgical treatment and were confirmed by postoperative pathology;one case failed to be diagnosed by puncture and was proved to be malignant by surgery.All the 56 cases were successfully sampled at one time(100.0%),with a total accuracy of 92.85%(52/56).None of the peripheral space-occupying lesions in ultrasound-guided percutaneous needle biopsy showed serious complications.Conclusion Ultrasound-guided coarse needle biopsy is a safe and effective method for the diagnosis of peripheral pulmonary space-occupying lesions.
