摘要
目的:探讨硒酵母联合左甲状腺素钠治疗甲状腺功能减退(甲减)的临床疗效。方法:选取重庆医科大学附属第三医院80例甲减患者,按随机数字表法将患者分为观察组和对照组,每组40例。对照组予以左甲状腺素钠治疗,观察组在对照组基础上予以硒酵母治疗。比较两组临床疗效、不良反应发生率、治疗前后血清促甲状腺激素(TSH)、游离三碘甲腺原氨酸(FT3)、游离四碘甲腺原氨酸(FT4)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)水平。结果:观察组治疗总有效率95. 00%,明显高于对照组的77. 50%(P<0. 05);治疗后,观察组血清TSH、TG、TC、LDL-C、TPOAb、TGAb水平均较对照组低,血清FT3、FT4、HDL-C水平较对照组高(P<0. 05)。两组不良反应发生率比较,差异无统计学意义(P>0. 05)。结论:硒酵母联合左甲状腺素钠治疗甲减患者,可增强治疗效果,调节血清甲状腺激素、血脂水平,降低甲状腺自身抗体水平,并具有较高安全性。
Objective: To analyze the clinical efficacy of selenium yeast combined with levothyroxine sodium in the treatment of hypothyroidism. Methods: A total of 80 patients with hypothyroidism in our hospital were randomized to receive levothyroxine sodium(control group,n=40)or levothyroxine sodium combined with selenium yeast(observation group,n=40). The clinical efficacy,incidence of adverse reactions,and the serum levels of thyroid stimulating hormone(TSH),free triiodothyronine(FT3),free tetraiodothyronine(FT4),triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),thyroglobulin antibody(TGAb)and thyroid peroxidase antibody(TPOAb)before and after treatment were compared between the two groups. Results: The total effective rate in the observation group was 95. 00%,which was higher than that in the control group(77. 50%)(P<0. 05). After treatment,the serum TSH,TG,TC,LDL-C,TPOAb and TGAb levels of the observation group were lower than those of the control group,while the serum FT3,FT4 and HDL-C levels were higher(P<0. 05). There was no significant difference in the incidence of adverse reactions between the two groups(P>0. 05). Conclusion: Selenium yeast combined with sodium levothyroxine in the treatment of hypothyroidism could enhance the therapeutic effect,regulate the level of serum thyroid hormone and blood lipid,reduce the level of thyroid autoantibodies,and was safe.
