摘要
全科医师是实现分级诊疗的基础和必要条件,培养合格的全科医师关键在于配备合格的带教师资。但如何培养合格的全科带教师资,保证全科住院医师规范化培训的质量,尚需要积极探索。浙江大学医学院附属邵逸夫医院全科住院医师规范化培训过程中非常重视基层实践基地师资的教学能力提升。通过多年实践,形成了独特的"医院与社区(Hospital-Community,H-C)一体化教学管理"模式,紧密的上下联动,一体化管理,通过分层递进的教学模式,社区师资由协同带教到独立带教的转变,持续提升社区师资的教学能力。社区师资通过参与医院全科的教学活动,不断提升全科思维模式和临床诊疗能力;通过医院师资下基层进行教学指导及各类形式的考核评估,提升社区师资门诊带教和理论授课的能力。该教学模式的实施,在提高本院师资教学水平的同时,也提升了基层实践基地全科带教师资的理论讲课水平及临床带教能力,为培养合格的全科医师奠定了基础。
The team of general practitioners is essential in realizing hierarchical health system, which is depended on sufficient and qualified trainers of general practitioners. However, how to train competent trainers of general practitioners to ensure the quality of residency training is still under development. The General Practice Department in Sir Run Run Hospital Affiliated to School of Medicine of Zhejiang University has attached significant importance to the teaching capacity building of trainers at gross-roots level and developed a unique "Hospital-Community(H-C) integrated teaching model". The "H-C teaching model" has created hierarchical progressive teaching model with close vertical flow and integrated management to transfer the role of general practice trainers in community health centers from integrated teaching to independent teaching, which further improved the clinical teaching capacity of community health centers. The thinking mode of general practice and clinical capacity of trainers in community health centers was strengthened through participating the teaching activities in general hospitals. Meanwhile, the capacity of outpatient teaching and theoretical teaching was also improved through guidance and assessments in various forms provided by trainers of general hospitals. The implementation of the "H-C integrated teaching model" improved the teaching capacity on general practice trainers in both general hospital and community health centers, which established the foundations for training qualified general practitioners.
