摘要
目的探讨冠心病患者胰岛素样生长因子结合蛋白-7(IGFBP-7)和心肌肌钙蛋白I(cTnI)的浓度及在冠心病诊断中的应用价值。方法选取2018年1-6月惠州市第六人民医院心内科收治的冠心病患者66例为冠心病组,包括急性心肌梗死(AMI)亚组20例、不稳定型心绞痛(UAP)亚组22例和稳定型心绞痛(SAP)亚组24例,选取同期年龄和性别匹配的体检健康者50例为对照组。酶联免疫吸附实验(ELISA)测定血清IGFBP-7浓度,时间分辨免疫荧光检测cTnI浓度。受试者工作特征曲线(ROC曲线)分析血清IGFBP-7和cTnI浓度对冠心病的诊断价值,Pearson相关分析血清IGFBP-7和cTnI浓度的相关性。结果冠心病患者血清IGFBP-7浓度明显高于对照组[(31.98±6.43)ng/mL vs.(21.98±7.55)ng/mL,P<0.05)];AMI亚组、UAP亚组和SAP亚组患者血清IGFBP-7浓度依次增加,各组间差异均有统计学意义(P<0.05);血清IGFBP-7浓度随心功能分级增高而升高,不同心功能分级间比较有统计学意义(P<0.05);当最佳临界值为27.43 ng/mL时,血清IGFBP-7浓度诊断CAD的ROC曲线下面积(AUC)为0.813(95%CI:0.739~0.912,P=0.000 8),灵敏度和特异度分别为79.9%和91.5%;血清cTnI浓度诊断冠心病AUC为0.845(95%CI:0.798~0.919,P=0.001 3);冠心病患者血清IGFBP-7和cTnI浓度呈显著正相关(r=0.761,P<0.05)。结论血清IGFBP-7和cTnI均可作为诊断冠心病有价值的指标,IGFBP-7对冠心病诊断价值高,可能为冠心病潜在血清标志物。
Objective To investigate the concentration of insulin-like growth factor binding protein-7(IGFBP-7)and cardiac troponin I(cTnI)in patients with coronary heart disease and their clinical values in the diagnosis of coronary heart disease.Methods From January to June 2018,66 patients with coronary heart disease were selected as the coronary heart disease group,including 20 patients with acute myocardial infarction(AMI),22 patients with unstable angina pectoris(UAP)and 24 patients with stable angina pectoris(SAP).The serum concentration of IGFBP-7 was determined by enzyme-linked immunosorbent assay(ELISA)and the concentration of cTnI was determined by time-resolved immunofluorescence.The diagnostic values of serum IGFBP-7 and cTnI levels for coronary heart disease were analyzed by receiver operating characteristic curve(ROC curve).The correlation between serum IGFBP-7 and cTnI levels was analyzed by pearson correlation.Results The serum concentration of IGFBP-7 in patients with coronary heart disease was significantly higher than that in control group[(31.98±6.43)ng/mL vs.(21.98±7.55)ng/mL,P<0.05)];the serum concentration of IGFBP-7 in AMI subgroup,UAP subgroup and SAP subgroup increased in turn,with significant differences among groups(P<0.05);the serum concentration of IGFBP-7 increased with the increase of cardiac function grade,and there were significant differences among different cardiac function grades(P<0.05);when the truncation value was 27.43 ng/mL,the area under ROC curve(AUC)of serum IGFBP-7 concentration for diagnosing CAD was 0.813(95%CI:0.739-0.912,P=0.000 8),sensitivity and specificity were 79.9%and 91.5%,respectively;serum cTnI concentration for diagnosing coronary heart disease was 0.845(95%CI:0.798-0.919,P=0.001 3);serum IGFBP-7 and cTnI concentration were positively correlated with coronary heart disease(r=0.761,P<0.05).Conclusion Serum IGFBP-7 and cTnI can be used as valuable indicators for the diagnosis of coronary heart disease.IGFBP-7 has high diagnostic value for coronary heart disease and may be a potential serum marker for coronary heart disease.
