Supreme喉罩和ProSeal喉罩在困难气道患者手术气道管理中的疗效对比观察 被引量:3

摘要 目的探讨Supreme喉罩、Pro Seal喉罩在困难气道患者手术气道管理中的应用效果及对麻醉质量的影响。方法回顾性选择2016年4月-2018年1月治疗的气管插管困难、需行全麻手术的困难气道患者92例作为对象,根据喉罩类型不同分为对照组(n=46例)与观察组(n=46例)。对照组给予Pro Seal喉罩,观察组采用Supreme喉罩,比较两组治疗成功率、拔出喉罩时间、气道封闭压、血流动力学水平。结果两组喉罩置入总成功率、苏醒时间比较均无统计学意义(P>0.05);观察组喉罩置入一次成功率,高于对照组(P<0.05);观察组拔除喉罩时间,短于对照组(P<0.05);观察组与对照组T1、T2及T3时间点PetCO2水平比较均无统计学意义(P>0.05);观察组T2、T3时间点气道密封压,均低于对照组(P<0.05);观察组与对照组置入喉罩前1 min、置入喉罩后1 min心率、MAP说比较均无统计学意义(P>0.05)。结论Supreme喉罩、Pro Seal喉罩用于困难气道患者手术气道管理中均能获得良好的效果,但是ProSeal喉罩气道密封性较高,而Supreme喉罩更易置入,应结合患者情况选择合适的喉罩。 Objective To investigate the effect of Supreme laryngeal mask and Pro Seal laryngeal mask on surgical airway management in patients with difficult airway and its effect on anesthesia quality.Methods 92 patients with difficult airway intubation who were treated with general anesthesia from April 2016 to January 1818 were selected retrospectively and were divided into control group(n=46 cases)and observation group(n=46 cases)according to the type of laryngeal mask.The Pro Seal mask was given to the control group,and the Supreme laryngeal mask was used in the observation group.The success rate,the time of pulling out the mask,the airway closing pressure,and the hemodynamic level between the two groups were compared.Results There were no significant differences in the total success rate and recovery time between the two groups(P>0.05).The success rate of the laryngeal mask in the observation group was higher than that in the control group(P<0.05).The hood time was shorter than that of the control group(P<0.05).There was no significant difference in the levels of PetCO2 between the observation group and the control group at the T1,T2 and T3 time points(P>0.05).The airway sealing pressure at the T2 and T3 time points in the observation group was observed and it was lower than the control group(P<0.05);There was no significant difference between the observation group and the control group 1 minute before the laryngeal mask was placed and 1 minute after the laryngeal mask was taken off(P>0.05).Conclusion Supreme laryngeal mask and Pro Seal laryngeal mask can achieve good results in surgical airway management for patients with difficult airway.ProSeal laryngeal mask has higher airway sealing,while Supreme laryngeal mask is easier to insert.The right laryngeal mask should be chosen according to the actual situation of patients.
