小剂量复方聚乙二醇电解质散剂联合二甲硅油散在便秘患者肠道准备中的应用 被引量:25

摘要 目的观察便秘患者行结肠镜检查前服用小剂量复方聚乙二醇电解质散剂联合二甲硅油用于肠道准备的临床效果。方法选取2015年8月至2018年1月在西安市第一医院消化内科接受结肠镜检查的240例便秘患者作为研究对象,依据随机数字法分为A、B、C三组,每组80例。其中,A组于检查前两天开始预先服用小剂量复方聚乙二醇电解质散(2大包+2小包溶于250 m L温水,每日2次),在此基础上服用2盒复方聚乙二醇电解质散及二甲硅油散(用温水配成30 m L溶液)准备肠道;B组于检查前两天开始服用小剂量复方聚乙二醇电解质散(2大包+2小包溶于250 m L温水,每日2次),在此基础上服用2盒复方聚乙二醇电解质散行肠道准备;C组单独服用2盒复方聚乙二醇电解质散剂准备肠道,比较3组患者的肠道准备充分率、肠腔气泡程度、操作时间、息肉检出率及不良反应发生率。结果A组患者的肠道准备充分率为85.0%(68/80),B组为81.2%(65/80),C组为65.4%(51/78),A组和B组的肠道准备充分率明显高于C组(P<0.05)。A组肠道气泡分级明显优于B组及C组,A组与B组进镜时间、退镜时间、操作总时间明显短于C组,A组直径<5 mm息肉和右半结肠息肉的检出率明显高于B组和C组(P<0.05)。3组患者恶心、呕吐、腹痛、腹胀等胃肠道不良反应发生率比较差异有统计学意义,且A组和B组的不良反应发生率明显低于C组(P<0.05),其中C组在肠道准备过程中有2例患者因发生肠梗阻而未能行结肠镜检查。结论便秘患者行结肠镜检查肠道准备中应用小剂量复方聚乙二醇电解质散剂联合二甲硅油散可安全、有效祛除肠腔气泡,获得满意的清洁肠道效果,有利于发现微小病变,对便秘患者结肠镜检查质量控制具有重要意义。 Objective To observe the clinical effect of low-dose compound polyethylene glycol electrolytes powder combined with dimethicone the bowel preparation of patients with constipation before colonoscopy.Methods A total of 240 patients with constipation undergoing colonoscopy in Xi’an NO.1 Hospital from Aug.2015 to Jan.2018 were included in the study,and randomly divided into group A,B and C,80 cases in each group.The group A was given low-dose compound polyethylene glycol electrolyte powder(2 big bags+2 small bags dissolved in 250 m L warm water,twice a day)two days before the examination,on this basis,two boxes of compound polyethylene glycol electrolyte powder and dimethicone powder(30 m L solution with warm water)were used to prepare intestinal tract;group B was also given low-dose compound polyethylene glycol electrolyte powder(2 big bags+2 small bags dissolved in 250 m L warm water,twice a day)two days before the examination,on this basis,two boxes of compound polyethylene glycol electrolyte powder were taken for intestinal preparation;the group C was given two boxes of compound polyethylene glycol electrolyte powder to prepare intestinal tract.The adequacy rate of intestinal preparation,degree of intestinal cavity bubbles,operation time,detection rate of polyps and incidence of adverse reactions were compared among the three groups.Results The intestinal preparation adequacy rate of group A was 85.0%(68/80),group B 81.2%(65/80),group C 65.4%(51/78)group A and group B significantly higher than that of group C(P<0.05).The classification of intestinal bubbles in group A was significantly better than that in group B and C.The time of entering,withdrawing and operating of group A and group B was significantly shorter than that in group C.The detection rate of<5 mm polyps and right hemicolonal polyps in group A was significantly higher than that in group B and group C(P<0.05).The incidence of gastrointestinal adverse reactions such as nausea,vomiting,abdominal pain and abdominal distension was significantly lower in group A and B than in group C(P<0.05).Two patients in group C failed to undergo colonoscopy because of intestinal obstruction during intestinal preparation.Conclusion Bowel preparation of low-dose polyethylene glycol electrolyte powder combined with dimethicone powder for patients with constipation can remove intestinal cavity bubbles safely and effectively,achieve satisfactory cleaning effect of intestinal tract,help to discover micro-lesions,and is of great significance to the quality control of colonoscopy for patients with constipation.
