摘要
目的探讨不同剂量丙泊酚在儿童全身麻醉中的临床效果。方法选取我院2016年1月~2018年10月收治的手术患儿80例为临床研究对象,根据丙泊酚的麻醉药物维持量分组,观察组[12ng/(kg·h)],对照组[8ng/(kg·h)];通过监测两组患儿各时刻点(T0、T1、T2、T3、T4)上的平均动脉压(MAP)与血浆血管紧张素Ⅱ(ANGⅡ)水平差异,探讨不同剂量丙泊酚在儿童全身麻醉中的临床应用效果,以确定小儿在全身麻醉中的丙泊酚用量标准。结果观察组患儿MAP水平与血浆ANGⅡ水平控制效果均优于对照组,观察组患儿仅在T4时刻点,生长激素水平略高于T0时刻点;对照组患儿在T2、T3、T4时刻点,生长激素水平均高于T0时刻点。结论12ng/(kg·h)的用药剂量可作为儿童全身麻醉中丙泊酚的标准用药剂量,其能够有效控制不同时刻点平均动脉压水平与血浆血管紧张素水平,安全性高,值得推广。
Objective To explore the clinical effects of different doses of propofol in general anesthesia in children.Methods Eighty children undergoing surgery who were admitted to our hospital from January 2016 to October 2018 were selected as clinical research objects.According to the maintenance dose of propofol,they were divided into the observation group 12ng/(kg·h)and the control group 8ng/(kg·h).By monitoring the difference in mean arterial pressure(MAP)and plasma angiotensin II(ANGII)levels at different time points(T0,T1,T2,T3,T4)between two groups,the clinical application effect of different doses of propofol in general anesthesia in children were explored to determine the standard of propofol use in general anesthesia in children.Results The control effect of MAP level and plasma ANGII level in the observation group was better than that of the control group.In the observation group,the growth hormone level at T4was slightly higher than that at T0.In the control group,the growth hormone levels at T2,T3 and T4were all higher than those at T0.Conclusion The dose of 12ng/(kg·h)can be used as the standard dose of propofol in general anesthesiain children.It can effectively control the mean arterial pressure level and plasma angiotensin level at different time points and it has high safety,which is worth promoting.
