摘要
目的探讨脑梗死患者行经颅多普勒超声(TCD)与颈部血管超声(CUS)联合诊断的临床效果。方法择取2016年8月~2019年1月期间我院收治的180例脑梗死患者为研究对象,分别为本组患者行TCD检查、CUS检查以及TCD+CUS检查,均行螺旋CT血管造影(CTA)/核磁共振动脉成像(MRA)检查,分别计算TCD检查、CUS检查以及TCD+CUS检查对脑梗死诊断准确率,分析具体诊断结果。结果TCD+CUS检查对脑梗死阳性、梗死分布和患者血流异常诊出率与CTA/MRA诊断结果差异无统计学意义(P>0.05),且显著高于TCD检查和CUS检查对脑梗死阳性、梗死分布和患者血流异常诊出率,差异有统计学意义(P<0.05),TCD检查对脑梗死阳性、梗死分布和患者血流异常诊出率与CUS检查结果相比差异无统计学意义(P>0.05)。结论TCD与CUS联合法对脑梗死患者诊断效果与影像学检查相近,有利于帮助临床医师对患者的病情进展、病灶分布有更好的了解,对其临床治疗具有指导意义,因此该种诊断方法值得用于临床推广。
Objective To explore the clinical effect of transcranial doppler ultrasound(TCD)combined with cervical vascular ultrasound(CUS)in the diagnosis of cerebral infarction.Methods One hundred and eighty patients with cerebral infarction admitted to our hospital from August 2016 to January 2019 were selected as the study subjects.TCD,CUS and TCD+CUS were performed in all patients.Spiral CT angiography(CTA)/magnetic resonance angiography(MRA)was performed in all patients.The diagnostic accuracy of TCD,CUS and TCD+CUS for cerebral infarction was calculated and the specific diagnostic results were analyzed.Results There was no significant difference between TCD+CUS and CTA/MRA in the diagnosis of positive cerebral infarction,distribution of infarction and abnormal blood flow(P>0.05),and it was significantly higher than that of TCD and CUS in the diagnosis of positive cerebral infarction,distribution of infarction and abnormal blood flow(P<0.05).There was no significant difference between TCD and CUS in the diagnosis of positive cerebral infarction,distribution of infarction and abnormal blood flow(P>0.05).Conclusion TCD combined with CUS has similar diagnostic effect with imaging examination in patients with cerebral infarction.It is helpful for clinicians to have a better understanding of patient's progress and distribution of lesions,and has guiding significance for clinical treatment.Therefore,this diagnostic method is worthy of clinical promotion.
