两种不同影像学方法对卵巢癌的诊断价值比较 被引量:14

摘要 目的比较计算机断层成像技术(CT)与核磁共振成像(MRI)两种不同影像学方法对卵巢癌的诊断价值。方法回顾性分析2016年3月至2018年6月台州市第一人民医院78例卵巢癌患者的临床资料,术前均行CT、MRI检查,以手术病理结果为金标准,对CT、MRI检查结果进行对照,进一步分析两种不同影像学方法对卵巢癌诊断效能的差异。结果根据术后病理结果,CT检查诊断卵巢癌的准确率为76.92%(60/78),MRI检查的准确率为91.03%(71/78),MRI诊断卵巢癌的准确率较CT检查明显升高(χ^2=4.76,P<0.05)。相比CT检查(69.23%),MRI检查诊断卵巢癌患者病理分期的准确率(87.18%)明显升高(χ^2=6.36,P<0.05)。术后12个月,卵巢癌复发及转移患者23例,MRI检查对卵巢癌复发和转移(肝脏等远处转移、腹膜后淋巴结转移、盆腔淋巴结转移、腹膜网膜转移、肠道转移)检出率显著高于CT检查(χ^2值分别为5.21、5.42、5.63、4.98、4.58,均P<0.05)。影像学图像显示方面,相比CT检查,MRI显示卵巢癌病灶细节方面更具优势,MRI对肿瘤分隔显示率、壁结节显示率、囊性病变显示率、实性病变显示率等均明显优于CT(χ^2值分别为9.976、5.711、8.443、6.276,均P<0.05)。结论 CT与MRI对卵巢癌的诊断各有优势,但相比CT,MRI在准确诊断卵巢癌、临床分期及复发转移的效能更高,对病灶细节的显示更有价值,因此具有较高的临床应用价值。 Objective To compare the value of computed tomography(CT) and magnetic resonance imaging(MRI) in ovarian cancer diagnosis.Methods The clinical data of 78 cases of ovarian cancer in our hospital from March 2016 to June 2018 were analyzed retrospectively.CT and MRI examinations were performed in all cases before operation.The results of two imaging examinations were compared and analyzed according to the golden standard of pathological results.Results According to the postoperative pathological results,the accuracy of CT and MRI in the diagnosis of ovarian cancer was 76.92%(60/78) and 91.03%(71/78),respectively.The results showed that the accuracy rate of MRI in diagnosing ovarian cancer was significantly higher than that in CT(χ^2=4.76,P<0.05).Compared with CT(69.23%),the accuracy of MRI in diagnosing pathological staging of ovarian cancer was significantly higher(87.18%)(χ^2=6.36,P<0.05).Twelve months after surgery,recurrence and metastasis of ovarian cancer were found in 23 cases,and the detection rate of ovarian cancer recurrence and metastasis(distant liver metastasis,retroperitoneal lymph node metastasis,pelvic lymph node metastasis,peritoneal omentum metastasis,intestinal metastasis) by MRI was significantly higher than CT examination(χ^2=5.21,5.42,5.63,4.98 and 4.58,respectively.P<0.05 for all).Compared with CT scanning,MRI was found to have more advantages in showing the details of the focus of ovarian cancer.MRI was superior to CT in the following:rates of displaying of tumor,wall nodule,cystic lesion and solid lesion(χ^2=9.976,5.711,8.443,6.276,respectively,P<0.05 for all).Conclusion CT and MRI have their own advantages in the diagnosis of ovarian cancer,but compared with CT,MRI is more effective in the accurate diagnosis of ovarian cancer,clinical staging,recurrence and metastasis,and is more valuable in the display of the details of the lesion,so it has a higher clinical value.
