摘要
目的研究窄谱中波紫外线(NB-UVB)治疗对寻常型银屑病患者外周血血清中人软骨糖蛋白-39(YKL-40)及脂肪因子腹腔脂肪型丝氨酸蛋白酶抑制剂(Vaspin)水平的影响,探讨YKL-40及Vaspin在银屑病发病机制中作用。方法试验组为40例进行期寻常型银屑病患者均采用NB-UVB照射治疗,用酶联免疫吸附试验(ELISA)法检测患者经NB-UVB照射前、后外周血血清中YKL-40及Vaspin的水平;选择30例健康体检者作为健康对照组。结果 40例患者治疗后银屑病皮损面积和严重程度指数(PASI)评分(3.26±1.24)显著低于治疗前(10.47±3.42),差异有统计学意义(t=17.55,P<0.05),NB-UVB治疗的有效率达88%(35/40)。治疗前,银屑病患者外周血清中YKL-40较对照组的表达量显著升高(t=3.48,P<0.05);外周血清中Vaspin较对照组的表达量显著降低(t=6.84,P<0.05);治疗前外周血血清中YKL-40水平与PASI评分呈正相关(r=0.97,P<0.05),Vaspin水平与PASI评分呈负相关(r=-0.98,P<0.05);患者外周血血清中YKL-40水平与Vaspin水平呈负相关(r=-0.95,P<0.05)。银屑病患者经NB-UVB治疗后,外周血血清中YKL-40表达量显著降低(t=20.5,P<0.05),Vaspin的表达量显著升高(t=8.33,P<0.05);治疗后患者外周血血清中YKL-40及Vaspin水平与健康对照组比较无统计学意义。结论 NB-UVB照射可以降低寻常型银屑病患者外周血血清中YKL-40水平,升高寻常型银屑病患者外周血血清中Vaspin水平;NB-UVB照射可能通过调节银屑病患者外周血YKL-40与Vaspin水平发挥治疗作用。
Objective In order to investigate the influences of narrow-band ultraviolet B(NB-UVB) on the levels of YKL-40 and Vaspin in peripheral blood of patients with psoriasis vulgaris,and to study the roles of YKL-40 and Vaspin in the pathogenesis of psoriasis. Methods The therapeutic method test group adopted NB-UVB radiation therapy on forty people with psoriasis vulgaris in active stage,and then via enzyme linked immunosorbent assay(ELISA) to compare the change of YKL-40 level and Vaspin level in peripheral blood serum before and after therapy. Another thirty healthy people were taken as control group. Results It showed that the PASI scoring(3.26±1.24) of the patients was much lower than that(10.47±3.42)before treatment,the differences(t=17.55,P<0.05) were statistically significant and the effective rate of NB-UVB therapy was 88%(35/40). Before treatment,the patients’ YKL-40 level(t=3.48,P<0.05) in peripheral blood serum was much higher than that of control group,it also indicated a positive correlation(r=0.97,P<0.05) with PASI scoring. There was a negative correlation between Vaspin level(r=-0.98,P<0.05) and PASI,and so was between YKL-40 level and Vaspin level(r=-0.95,P<0.05). After treatment,the YKL-40 level(t=20.5,P<0.05) in peripheral blood serum dropped greatly while the Vaspin level(t=8.33,P<0.05) increased greatly,yet the set of data in comparison with that of the cotnrolled group had no statistical significance. Conclusion The NB-UVB radiation therapy takes effects through lowering YKL-40 level and increasing the Vaspin level in peripheral blood serum in psoriasis vulgaris.
