生物反馈联合电刺激对盆腔器官脱垂患者尿动力及SOD、MDA水平的影响 被引量:15

Effect of biofeedback combined with electrical stimulation on the urinary motility, SOD and MDA levels in patients with pelvic organ prolapse

摘要 目的探讨生物反馈联合电刺激对盆腔器官脱垂患者尿动力及SOD、MDA水平的影响。方法根据随机双盲法选取2016年1月至2019年1月在黄冈市中心医院接受治疗的180例盆腔器官脱垂患者作为研究对象。将这180例患者分为对照组(90例)和观察组(90例)。对照组患者接受盆底肌肉训练,观察组患者实施生物反馈联合电刺激疗法,共治疗6周。对比两组患者疗效、6个月内复发率,并观察治疗前后患者盆腔器官脱垂程度、尿动力及SOD、MDA水平变化。结果观察组患者治疗总有效率比对照组患者高(91.11%VS 78.89%),复发率比对照组患者低(2.22%VS 11.11%),差异具有统计学意义(P<0.05);观察组患者治疗后盆腔器官脱垂程度轻于治疗前,差异有统计学意义(P<0.05);观察组患者治疗后平均尿流率、最大膀胱容量比对照组患者高,最大逼尿肌压、残余尿量比对照组患者低,差异具有统计学意义(P<0.05);观察组患者治疗后SOD水平比对照组患者高,MDA水平比对照组患者低,差异具有统计学意义(P<0.05)。结论针对盆腔器官脱垂患者实施生物反馈联合电刺激可提升其疗效、降低复发,减轻盆腔器官脱垂程度,改善尿动力学,且可降低氧化应激反应,提高机体清除氧自由基的能力,减轻组织损伤程度,其机制可能与血清SOD、MDA水平有关,为盆腔器官脱垂的防治研究提供了新的方向基础。 Objective To investigate the effects of biofeedback combined with electrical stimulation on the urinary motility,SOD and MDA levels in patients with pelvic organ prolapse.Methods 180 patients with pelvic organ prolapse treated in our hospital from January 2016 to January 2019 were randomly divided into the control group(90 cases)and the observation group(90 cases).The control group received pelvic floor muscle training,and the observation group received biofeedback combined with electrical stimulation therapy,both for 6 weeks.The curative effect of the two groups and the recurrence rate within 6 months were compared.The changes in pelvic organ prolapse,urinary motility,SOD and MDA levels were observed before and after treatment.Results The total effective rate of the observation group was higher than that of the control group(91.11%vs 78.89%),and the recurrence rate was lower than that of the control group(2.22%vs 11.11%),with statistically significant differences(P<0.05).The degree of pelvic organ prolapse in the observation group after treatment was lighter than that before treatment(P<0.05).The average urinary flow rate and maximum bladder volume of the observation group were higher than those of the control group,and the maximum detrusor pressure and residual urine volume were lower than those in the control group,with statistically significant differences(P<0.05).The level of SOD in the observation group was higher than that in the control group,and the level of MDA was lower than that in the control group,with statistically significant differences(P<0.05).Conclusions Biofeedback combined with electrical stimulation for patients with pelvic organ prolapse can improve the curative effect,urodynamics and the ability of scavenging oxygen free radicals and reduce the recurrence,the degree of pelvic organ prolapse,oxidative stress response and tissue damage.The mechanism may be related to serum SOD and MDA levels,which provides a new direction for the prevention and treatment of pelvic organ prolapse.
