益肾化瘀降浊方对腹膜透析患者肾纤维化TGF-β1/Smad信号通路影响的研究 被引量:4

Study on the effect of Yishen Huayu Jiangzhuo Decoction on TGF-β1/Smad signaling pathway in renal fibrosis in peritoneal dialysis patients

摘要 目的研究益肾化瘀降浊方对腹膜透析肾衰竭患者肾纤维化的影响及可能机制。方法选取60例初次进行腹膜透析且中医辨证为脾肾亏虚、血脉瘀滞证肾衰竭患者,随机分为治疗组30例、对照组30例,均行腹膜透析治疗,治疗组同时服用益肾化瘀降浊方,每4周为1个疗程,共服6个疗程。观察2组治疗前后中医证候评分及血尿素氮(BUN)、血肌酐(SCr)、转化生长因子β1(TGF-β1)及Smad2水平变化,比较2组综合疗效和中医疗效。结果2组治疗后中医证候积分及血BUN、SCr、TGF-β1、Smad2水平均明显降低(P均<0.05),且治疗组均明显低于对照组(P均<0.05);治疗组综合疗效和中医疗效总有效率均为86.7%(26/30),对照组分别为66.7%(20/30)和63.3%(19/30),治疗组均明显高于对照组(P均<0.05)。结论益肾化瘀降浊方可通过降低血清TGF-β1、Smad2水平延缓腹膜透析患者肾纤维化。 Objective It is to study the effect of Yishen Huayu Decoction on renal fibrosis in patients treated with peritoneal dialysis and its possible mechanism.Methods Sixty patients with renal failure who underwent peritoneal dialysis for the first time and their TCM syndrome differentiation was syndrome of spleen and kidney deficiency and blood stasis were randomly divided into treatment group(n=30)and control group(n=30).All patients underwent peritoneal dialysis.The treatment group was treated with Yishen Huayu Jiangzhuo Decoction at the same time.Every 4 weeks as one treatment course,both groups were treated for a total of 6 courses.The changes of scores of TCM syndromes and the levels of blood urea nitrogen(BUN),serum creatinine(SCr),transforming growth factor-β1(TGF-β1)and Smad2 were observed before and after treatment,and the comprehensive curative effect and the curative effect of TCM syndrome were compared in the two groups.Results The scores of TCM syndrome and the levels of blood BUN,SCr,TGF-β1 and Smad2 in the two groups were significantly decreased after treatment(P<0.05),and these indexes in the treatment group were significantly lower than those in the control group after treatment(P<0.05).The total effective rate of comprehensive curative effect and the curative effect of TCM syndrome were both 86.7%(26/30)in the treatment group,and were 66.7%(20/30)and 63.3%(19/30),respectively in the control group.The rates in the treatment group were significantly higher than those in the control group(P<0.05).Conclusion Yishen Huayu Decoction can delay renal fibrosis in peritoneal dialysis patients by down-regulating the levels of serum TGF-β1 and Smad2.
