摘要
目的探讨多b值DWI对人类免疫缺陷病毒(human immunodeficiency vi rus,HIV)阳性患者的诊断价值。方法选取我院2017年1月~2018年12月收治的HIV感染阳性患者36例作为研究组进行研究,另取36例健康者作为对照组。两组均进行多b值DWI检测,观察患者DWI检测结果并使用单指数模型、双指数模型及拉伸指数模型进行分析,通过检测结果对患者进行诊断。比较两组ADC、DDC、ADCf、ADCs及α水平,观察b值DWI检测阳性预测值、阴性预测值、诊断灵敏度、特异度及约登指数。结果两组额叶双侧ADC、DDC、ADCs水平及右侧ADCf、α值差异不显著(P>0.05),研究组左侧ADCf、α值显著低于对照组(P<0.05);经DWI检测,对照组检出阳性4例,阴性32例,研究组检出阳性33例,阴性3例,即真阳性33例。真阴性32例,假阳性4例,假阴性3例。诊断灵敏度91.67%,特异度88.89%。阳性预测值89.19%,阴性预测值91.43%,约登指数80.62%。结论HIV感染阳性切实会引起患者DWI检测结果改变,使用多b值DWI进行检测诊断,具有较高敏感性及特异性,具有一定诊断价值,为临床HIV阳性的确诊能提供一定依据。
Objective To investigate the diagnostic value of multi-b value DWI in human immunodeficiency virus(HIV)-positive patients.Methods 36 HIV-positive patients who were admitted to our hospital from January 2017 to December 2018 were selected in the study group.Another 36 healthy subjects were assigned to the control group.Both groups were given multiple b-value DWI tests.The patient's DWI test results were observed and analyzed using a single exponential model,a double exponential model,and a tensile index model.The patient was diagnosed by the test results.The levels of ADC,DDC,ADCf,ADCs andαwere compared between the two groups,and the b-value DWI was observed to test the positive predictive value,negative predictive value,diagnostic sensitivity,specificity and Yoden index.Results There were no significant differences in the levels of ADC,DDC,ADCs on both sides of frontal lobes and ADCf andαvalue on the right side between the two groups(P>0.05).The ADCf andαvalues on the left side in the study group were significantly lower than those in the control group(P<0.05);after DWI test,there were 4 positive cases and 32 negative cases in the control group.There were 33 positive cases and 3 negative cases in the study group.That is,33 cases were true positive,32 cases were true negative,4 cases were false positive,and 3 cases were false negative.The diagnostic sensitivity was 91.67%and the specificity was 88.89%.The positive predictive value was 89.19%,the negative predictive value was 91.43%,and the Yoden index was 80.62%.Conclusion Positive HIV infection will actually cause changes in DWI test results.The use of multi-b value DWI for detection and diagnosis has a high sensitivity and specificity,and has a certain diagnostic value,which provides a basis for the diagnosis of clinical HIV positive cases.
