摘要
目的探讨骨质疏松性股骨侧假体周围骨折的因素与治疗。方法选取2013年1月至2018年12月我院收治的42例骨质疏松性股骨侧假体周围骨折患者作为研究对象,分析患者骨折的因素,并对患者实施钛缆钢板结合抗骨质疏松治疗,进行患者随访观察骨折恢复情况。结果假体周围骨折的病因主要包括两个方面,一是局部因素,包括假体松动或骨溶解、术后内置物周围发生局部骨质疏松,二是全身因素,包括长期卧床导致的骨质疏松、妇女绝经导致的骨质疏松或老年性骨质疏松,除此之外,患者的年龄、性别、假体类型、固定方式等都是假体周围骨折的危险因素;对42例接受治疗的患者进行随访,所有患者骨折均得到愈合,未发生感染、假体松动等不良现象。结论假体松动和骨质疏松是导致假体周围骨折的主要因素,对股骨侧假体周围骨折患者实施钛缆钢板结合抗骨质疏松治疗,患者骨折恢复情况较好,治疗效果明显。
Objective To investigate the factors and treatment of osteoporotic periprosthetic femoral fractures.Methods from January 2013 to December 2018,42 patients with osteoporotic femoral lateral prosthesis fractures admitted to our hospital were selected as the research objects.The factors of fractures were analyzed,and the patients were treated with titanium cable plate combined with anti-osteoporosis treatment.The patients were followed up to observe the fracture recovery.Results The causes of periprosthetic fracture mainly include two aspects:one is local factors,including prosthetic loosening or osteolysis,local osteoporosis around implants after operation,and the other is systemic factors,including osteoporosis caused by long-term bed rest,osteoporosis caused by menopause or senile osteoporosis.In addition,the age,sex,type of prosthesis and fixation method of the patients.All of them were risk factors for periprosthetic fracture;42 patients were followed up and all the fractures were healed without any adverse phenomena such as infection and prosthetic loosening.Conclusion prosthesis loosening and osteoporosis are the main factors leading to periprosthesis fracture.In patients with femoral lateral periprosthesis fracture,titanium cable plate combined with anti-osteoporosis treatment has a good fracture recovery and obvious treatment effect.
