摘要 目的:探究支气管镜下冷冻治疗支气管结核(endobronchial tuberculosis,EBTB)的临床应用。方法:研究对象为本院2018年1月-2019年1月收治的85例EBTB患者。按治疗方案分A组(n=40,常规抗结核治疗)与B组(n=45,常规抗结核联合冷冻治疗)。记录B组冷冻治疗次数及并发症发生率。比较两组临床疗效、症状及免疫功能情况。结果:B组冷冻次数为2~6次,平均(3.97±0.52)次;治疗期间未见出血、穿孔等并发症发生,均顺利完成治疗。B组治疗总有效率明显高于A组(P<0.05)。治疗后两组气道直径、气促指数、第一秒最大呼气量(FEV1)均明显优于治疗前,且B组气道直径、气促指数及FEV1均优于A组(P<0.05)。治疗后两组CD3^+、CD4^+、CD4^+/CD8^+均高于治疗前,CD8^+低于治疗前,B组CD3^+、CD4^+、CD4^+/CD8^+均明显高于A组,CD8^+明显低于A组(P<0.05)。结论:支气管镜下冷冻治疗EBTB效果明显、安全性良好,可明显改善症状及免疫功能,值得临床推广。 Objective: To explore clinical application of bronchoscopic cryotherapy in treatment of endobronchial tuberculosis(EBTB). Method: The study subjects were 85 EBTB patients admitted to the hospital from January 2018 to January 2019. According to different treatment plans, they were divided into group A(n=40, routine treatment) and group B(n=30, routine treatment combined with cryotherapy). The number of cryotherapy and incidence of complications during treatment in group B were recorded. The clinical curative effect, symptoms and immune function were compared between the two groups. Result: The number of cryotherapy in group B was 2-6 times, with an average of(3.97±0.52) times. There were no complications such as hemorrhage and perforation during treatment. The treatment was successfully completed for group B. The total effective rate of group B was significantly higher than that of group A(P<0.05). After treatment, airway diameter, polypnea index and forced expiratory volume in 1 second(FEV1) in both groups were significantly better those before treatment, and airway diameter, polypnea index and FEV1 in the group B were better than those in the group A(P<0.05). After treatment, CD3^+, CD4^+ and CD4^+/CD8^+ in both groups were higher than those before treatment, while CD8^+ was lower than that before treatment, and CD3^+, CD4^+ and CD4^+/CD8^+ in group B were significantly higher than those in group A, while CD8^+ was significantly lower than that in group A(P<0.05). Conclusion: The effect of bronchoscopic cryotherapy on EBTB is obvious, with good safety, it can obviously improve the symptoms and immune function, and is worthy of clinical promotion.
