跗骨窦切口微创钢板及空心钉轴向内固定联合术后低分子肝素治疗跟骨骨折的效果及对血清sVCAM-1、HMGB1水平的影响 被引量:10

摘要 目的探讨跗骨窦切口微创钢板及空心钉轴向内固定联合术后低分子肝素治疗跟骨骨折的效果。方法将2016年2月-2017年4月石家庄市第三医院创伤骨科收治的120例跟骨骨折患者纳入研究,以随机数字表法分为两组,每组60例,分别使用跗骨窦切口微创钢板及空心钉轴向内固定联合低分子肝素治疗(联合组)和常规内固定治疗(常规组),比较治疗效果、Gissane角、Bohler角、凝血功能、血清可溶性细胞间黏附分子1(sVCAM-1)、高迁移率族蛋白1(HMGB1)之间的差异。结果治疗后,两组治疗效果及不良反应差异无统计学意义;两组Gissane角、Bohler角均有明显提升,联合组APTT、PT高于常规组[(39.78±2.56)s vs.(36.18±2.17)s、(24.52±0.82)s vs.(19.32±1.13)s](P<0.05);血清sVCAM-1、HMGB1低于常规组[(467.82±30.25)ng/mL vs.(492.26±30.29)ng/mL、(3.16±1.55)μg/L vs.(6.46±1.62)μg/L](P<0.05)。结论跗骨窦切口微创钢板及空心钉轴向内固定联合术后低分子肝素治疗跟骨骨折患者,血清sVCAM-1、HMGB1水平降低,疗效显著。 Objective To investigate the effect of minimally invasive plate and hollow screw internal fixation combined with low molecular weight heparin on calcaneal fracture after tarsal sinus incision.Methods From Apr.2017 to Feb.2016,120 patients with calcaneal fractures were admitted into Department of Orthopedics Trauma of Shijiazhuang the Third Hospital.They were randomly divided into combined group and normal group,with 60 cases in each group.Patients in the combined group were treated by minimally invasive steel plate and cannulated screw internal fixation through the tarsal sinus incision combined with low molecular heparin therapy and patients in the normal group were treated by conventional internal fixation.The treatment effect,Gissane Angle,Bohler Angle,coagulation function,serum soluble intercellular adhesion molecule 1(sVCAM-1),APTT,PT,and high mobility group protein 1(HMGB1)were compared between the two groups.Results After treatment,there was no significant difference in the treatment effect and adverse reactions between the two groups;the Gissane angle and Bohler angle of the two groups were significantly increased,APTT and PT in the observation group were higher than those in the control group(P<0.05);serum sVCAM-1 and HMGB1 were lower than those in the control group(P<0.05).Conclusion The level of sVCAM-1 and HMGB1 decreased significantly after operation in serum of patients with calcaneal fracture treated by minimally invasive plate and hollow screw internal fixation combined with low molecular weight heparin.
