摘要
目的探讨血清胰岛素样生长因子1(IGF-1)、血脂与2型糖尿病(T2DM)中医辨证分型的关系及临床意义。方法根据证型不同,将2014年至2015年58例2型糖尿病患者分为燥热伤肺证(30例)及脾气亏虚证(28例)两个证型组。选择同期无T2DM者28例为对照组,对两个证型组和对照组(28例)进行空腹血糖(FBG)、胰岛素敏感性指数(ISI)、IGF-1、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平的检测。结果 ISI、IGF-1、HDL-C水平以对照组→燥热伤肺证组→脾气亏虚证组之序递减,TG水平以此序递增(P<0.05)。燥热伤肺证组和脾气亏虚证组FBG水平显著高于对照组(P<0.05),但此两组间FBG水平比较差异无统计学意义(P>0.05)。经偏相关分析发现,IGF-1与ISI具有相关性。结论血清IGF-1和血脂的异常与T2DM有关,脾气亏虚证是T2DM胰岛素敏感性异常的关键病机。
Objective To investigate the relationship between serum insulin-like growth factor 1(IGF-1),lipid and Chinese medicine syndrome differentiation of type 2 diabetes mellitus(T2 DM) and its clinical significance.Methods A total of 58 patients with T2 DM who received treatment from 2014 to 2015 were selected,and they were divided into dry-heat impairing lung syndrome group(n=30) and deficiency of the spleen Qi syndrome group(n=28) according to the different type of syndrome,and 28 people without T2 DM in the same period were selected as control group.The levels of fasting blood glucose(FBG),insulin sensitivity index(ISI),IGF-1,triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C) and low density lipoprotein cholesterol(LDL-C) were compared between the two syndromes group and control group.Results In the order of control group,dry-heat impairing lung syndrome group and deficiency of the spleen Qi syndrome group,the levels of ISI,IGF-1,HDL-C decreased and the level of TG increased(P<0.05).The FBG of dry-heat impairing lung syndrome group and deficiency of the spleen Qi syndrome group were significantly higher than that of control group(P<0.05),but there was no difference between the two groups(P>0.05).LDL-C in deficiency of the spleen Qi syndrome group was higher than that in control group(P<0.05).There was no significant difference in TC among three groups(P>0.05).The correlation analysis showed that IGF-1 was correlated with ISI.Conclusion The abnormalities of serum IGF-1 and lipid are related to T2 DM.Deficiency of the spleen Qi syndrome is the key pathogenesis of insulin sensitivity abnormality in T2 DM.
