摘要
目的探讨组织多普勒技术应用于糖尿病性心肌病患者右室功能评价的临床价值。方法选取2017年8月—2019年1月该院收治的糖尿病性心肌病患者32例为研究组,另选取同时期在该院健康体检者25名为对照组,对两组患者均进行常规超声心动图检查,并应用脉冲多普勒对三尖瓣口舒张期早期峰值流速(E)、晚期峰值流速(A)进行检测,并计算E/A,应用M型超声对右室侧壁三尖瓣环收缩期位移(TAPSE)进行测量,应用组织多普勒技术分析右心室侧壁的三尖瓣环处舒张期早期运动速度(Ve)、晚期运动速度(Va)、收缩期运动速度(Vs)、等容收缩期速度(IVCv),并对Ve/Va和IVCv/Vs进行计算。结果研究组患者右心室Ve/Va值显著低于对照组(P<0.05),其他参数对比差异无统计学意义(P>0.05);研究组右心室舒张期IVCv、IVCv/Vs值显著高于对照组,Vs值显著低于对照组(P<0.05),两组受试者右心室舒张期RVEF、TAPSE值对比差异无统计学意义(P>0.05);组织多普勒技术对右心室舒张功能异常检出率显著高于脉冲多普勒检查(P<0.05)。结论组织多普勒技术应用于糖尿病性心肌病患者右室功能评价可有效提高右室功能检出率,有助于临床早期诊断糖尿病性心肌病。
Objective To investigate the clinical value of tissue Doppler in the evaluation of right ventricular function in patients with diabetic cardiomyopathy.Methods A total of 32 patients with diabetic cardiomyopathy admitted to the hospital from August 2017 to January 2019 were enrolled in the study group.25 patients in the same period in our hospital were selected as the control group.Cardiac examination,and the application of pulse Doppler to detect the early peak flow velocity(E)and late peak flow velocity(A)of the tricuspid valve diastolic phase,and calculate the E/A,and apply the M-mode ultrasound to the right ventricular side wall tricuspid annulus systolic displacement(TAPSE)was measured.Tissue Doppler technique was used to analyze the early diastolic velocity(Ve),late motion velocity(Va),and systolic velocity(Vs)of the tricuspid annulus of the right ventricular wall,isovolumic systolic velocity(IVCv),and calculations for Ve/Va and IVCv/Vs.Results The value of Ve/Va in the right ventricle of the study group was significantly lower than that of the control group(P<0.05).There was no statistically significant difference in other parameters(P>0.05).The values of IVCv and IVCv/Vs in the right ventricular diastolic phase of the study group were significantly higher than those in the control group.The values were significantly lower than the control group(P<0.05).There was no statistically significant difference in right ventricular diastolic RVEF and TAPSE between the two groups(P>0.05).The detection rate of right ventricular diastolic dysfunction was significantly higher by tissue Doppler technique,higher than pulse Doppler examination(P<0.05).Conclusion The application of tissue Doppler in the evaluation of right ventricular function in patients with diabetic cardiomyopathy can effectively improve the detection rate of right ventricular function and contribute to the early diagnosis of diabetic cardiomyopathy.
