摘要
目的探讨代谢因素与尿路上皮癌病理分级、分期的相关性。方法收集2017年1月1日至2018年3月31日潍坊市人民医院收治的术后病理证实为原发性尿路上皮癌的患者292例,回顾性分析其临床资料中代谢因素(包括血糖、血清甘油三酯、胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇和血尿酸)与尿路上皮癌病理分级、分期的相关性。统计学方法采用卡方检验、多因素Logistic回归分析。结果292例患者中122例(41.80%)为低级别尿路上皮癌,170例(58.20%)为高级别尿路上皮癌;非肌层浸润性尿路上皮癌患者123例(42.10%),肌层浸润性尿路上皮癌患者169例(57.90%)。糖尿病、血脂异常与尿路上皮癌的病理分级、分期均相关,差异均有统计学意义(P<0.05);而高尿酸血症仅与尿路上皮癌的病理分级相关,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,糖尿病、血脂异常与尿路上皮癌的分级(OR=2.844,P<0.05;OR=3.141,P<0.05)和分期(OR=2.580,P<0.05;OR=2.532,P<0.05)均相关,而高尿酸血症仅与尿路上皮癌的病理分级相关(OR=4.942,P<0.05)。结论代谢紊乱与尿路上皮癌的进展相关,尿路上皮癌合并糖尿病、血脂异常患者的病理分级、分期更高,而合并高尿酸血症的患者仅病理分级较高,与病理分期无明显相关性。
Objective To investigate the relevance between metabolic factors and the pathological grade and stage in urothelial carcinoma patients.Methods The data of 292 primary urothelial carcinoma patients who underwent surgery and pathological examination in Weifang People's Hospital from January 1 st,2017 to March 31 st,2018 were retrospectively collected.Metabolic factors were collected from the clinical data,including fasting blood glucose,serum total triglyceride,serum total cholesterol,serum low density lipoprotein cholesterol,serum high density lipoprotein cholesterol and serum uric acid.Statistics analysis of χ^2 test and Logistic regression were used to investigate the relevance between metabolic factors and the pathological grade and stage in urothelial carcinoma patients.Results There were a total of 292 urothelial carcinoma patients,including 122(41.80%)low grade,170(58.20%)high grade,123(42.10%)nonmuscle invasion,and 169(57.90%)muscle invasion,respectively.Diabetes and dyslipidemia were associated with pathological grade and stage of urothelial carcinoma,and the difference was significant(P<0.05),but hyperuricemia was just associated with pathological grade of urothelial carcinoma,and the difference was significant(P<0.05).The results of Logistic regression analyses showed that,diabetes and dyslipidemia were associated with pathological grade(OR=2.844,P<0.05;OR=3.141,P<0.05)and stage(OR=2.580,P<0.05;OR=2.532,P<0.05)of urothelial carcinoma,but hyperuricemia was just associated with pathological grade(OR=4.942,P<0.05)of urothelial carcinoma.Conclusions Metabolic disorders were associated with the progression of urothelial carcinoma.Patients with diabetes and dyslipidemia were found to have significantly higher grade and stage of urothelial carcinoma.Patients with hyperuricemia were found to have significantly higher grade,but not related with the pathological stage of urothelial carcinoma.
