实时荧光定量PCR法检测肾移植术后BK病毒和JC病毒感染及临床意义

摘要 目的检测肾移植患者术后BK病毒DNA和JC病毒DNA并探讨其临床应用价值。方法应用实时荧光定聚合酶链反应(PCR)对广东省第二人民医院125例同种异体肾移植患者术后血液及尿液标本分别进行BK病毒DNA和JC病毒DNA检测。结果125例肾移植患者中,病毒尿症阳性共35例(28.0%),其中尿BK病毒DNA阳性25例(20.0%)、尿JC病毒DNA阳性10例(8.0%);病毒血症有20例(16.0%),其中血液BK病毒DNA阳性20例(16.0%)、JC病毒血症未检出;BK病毒DNA血液和尿液均为阳性有5例(4.0%),并经肾脏穿刺病理活检确证为BK病毒相关性肾病(BKVAN)。结论尿液、血液中的BK病毒和JC病毒检测能够早期发现BK、JC病毒感染,为预防肾移植术后多瘤病毒相关性肾病(PVAN)提供重要的依据。 Objective To detect BK virus and JC virus infection in renal transplant patients and to explore the clinical applica?tion.Methods The study included 125 kidney transplant recipients in the hospital.The BK-DNA and JC-DNA load in urine and plas?ma were detected by using real-time PCR.Result Of 125 recipients,there were 35 cases(28.0%)of virus,the positive rate of BK vi?rus was 20.0%and the positive rate of JC virus was 8.0%.Virus was detected in 20 recipients(16.0%)and the positive rate of BK vi?rus was 16.0%.No JC virus was detected.BKDAN was identified in 5 patients(4.0%)by pathology test from the groups with BK virus and BKVAN.Conclusion To make early diagnosis of BK virus and JC virus infection,detection of BK-DNA and JC-DNA in urine and plasma samples is important,which provides an important basis for the prevention of PVAN.
