摘要
目的探讨冠状动脉64层螺旋CT血管成像对冠心病患者的临床诊断价值。方法选取2017年1月—2018年12月期间于韶关市第三人民医院就诊的102例疑似冠心病患者,所有患者均需接受冠状动脉64层螺旋CT血管成像检查,记录诊断结果,并以冠状动脉造影作为金标准,分析诊断效能与管壁腔钙化情况。结果冠状动脉64层螺旋CT血管成像对冠心病的诊断敏感度为95.65%,特异度为96.97%,阳性预测值为98.51%,阴性预测值为91.43%,准确度为96.08%;102例患者应显示1530段血管分支,冠状动脉64层螺旋CT血管成像实际显示1485段,显示成功率为97.06%;在1530段血管分支中,正常153段,占10.00%,轻度狭窄670段,占43.79%,中度狭窄530段,占34.64%,重度狭窄177段,占11.57%,且其对应混合斑块率分别为38.56%、27.01%、73.96%、58.76%。结论冠状动脉64层螺旋CT血管成像为临床诊断冠心病的有效手段,其属于无创诊断,具有简便安全、可重复等优势,利于患者接受。
objective To investigate the clinical diagnostic value of 64-slice spiral CT angiography of coronary artery in patients with coronary heart disease.Methods 102 patients with suspected coronary heart disease admitted to the hospital from January,2017 to December,2018 were selected.All patients were required to receive 64-slice spiral CT angiography of the coronary artery to record the diagnosis results,and coronary angiography was used as the gold standard to analyze the diagnostic efficacy and calcification of the ves?sel wall cavity.Results The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of 64-slice spi?ral CT angiography in the diagnosis of coronary heart disease were 95.65%,96.97%,98.51%,91.43%and 96.08%,respectively.102 patients should show 1530 segments of vascular branches,and the 64 slice spiral CT angiography of coronary artery actually shows 1485 segments,with a display success rate of 97.06%.Among the 1530 vascular branches,153 were normal,accounting for 10.00%;670 were mild stenosis,accounting for 43.79%;530 were moderate stenosis,accounting for 34.64%;177 were severe stenosis,ac?counting for 11.57%;and the corresponding mixed plaque rates were 38.56%,27.01%,73.96%and 58.76%,respectively.Conclu?sion 64-slice spiral CT angiography of coronary artery is an effective method for clinical diagnosis of coronary heart disease.
