淫羊藿总黄酮联合低频脉冲电磁场治疗骨质疏松性桡骨远端骨折 被引量:8

Treatment of osteoporotic distal radius fracture with total flavonoids of epimedium combined with low-frequency pulsed magnetic fields

摘要 目的研究淫羊藿总黄酮联合低频脉冲电磁场(LFPMF)在骨质疏松性桡骨远端骨折患者中的治疗效果。方法选取120例骨质疏松性桡骨远端骨折患者为研究对象,随机分为观察组和对照组,每组各60例。对照组患者采用常规手法复位联合LFPMF治疗;观察组在对照组基础上加用淫羊藿总黄酮胶囊,每次2粒,每日3次,7d为一个疗程。所有患者均治疗6个疗程。比较2组患者的临床疗效、骨密度(BMD)与骨代谢相关指标水平及不良反应发生情况。结果观察组治疗总有效率显著高于对照组(92%vs. 75%, P <0.05),其愈合时间显著短于对照组[(11.4±3.1)周vs.(13.6±2.8)周,P <0.01]。治疗后,2组患者的健侧桡骨BMD、患侧桡骨BMD、骨钙素、Ⅰ型前胶原羧基端前肽及骨源性碱性磷酸酶水平均显著高于治疗前(P<0.01),且观察组显著高于对照组(P<0.05, P<0.01)。2组患者的不良反应发生率无显著差异(P>0.05)。结论淫羊藿总黄酮联合LFPMF可提高骨质疏松性桡骨远端骨折患者临床疗效,改善患者BMD及骨代谢水平,安全性高。 AIM To study the therapeutic effect of total flavonoids of epimedium combined with low-frequency pulse magnetic fields( LFPMF) on patients with osteoporotic distal radius fracture. METHODS A total of 120 patients with osteoporotic distal radius fracture were selected as research objects and randomly divided into observation group( n =60) and control group( n =60). The control group was treated with conventional manipulations combined with LFPMF, while the observation group was treated with total flavonoids of epimedium(2 pills per time, tid, 7 days for a course of treatment) based on the control group. All patients were treated for 6 courses of treatment. The clinical effect, the levels of bone mineral density(BMD) and bone metabolism, and the adverse reactions were compared between the two groups. RESULTS The total effective rate of the observation group was significantly better than that of the control group(92% vs. 75%, P < 0.05), and the healing time of the observation group was significantly lower than that of the control group((11.4 ± 3.1) weeks vs.( 13.6 ± 2.8) weeks, P < 0.01). After the treatment, the level of contralateral radius BMD, same radius BMD, osteocalcin, procollagen type 1 carboxy-teriminal propeptide and bone alkaline phosphatase in the two groups were significantly higher than before treatment(P < 0.01), and the observation group was significantly higher than the control group( P < 0.05, P < 0.01). There was no significant difference in the incidence of adverse reactions between the two groups( P > 0.05). CONCLUSION Total flavonoids of epimedium combined with LFPMF can increase the clinical efficacy and improve the level of BMD and bone metabolism of patients with osteoporotic distal radius fracture with high safety.
