摘要
探讨甲状腺穿刺液基细胞学检查在亚急性甲状腺炎和桥本氏甲状腺炎鉴别诊断中的价值。对37例初诊的亚急性甲状腺炎患者和34例诊断为桥本氏甲状腺炎的患者进行甲状腺功能及相关抗体检查、甲状腺穿刺液基细胞学检查,两组病例甲状腺功能及甲状腺相关抗体指标中的FT3、FT4、TSH、TGAb、TPOAb有着显著差异(P <0. 01),而TRAb则差异不显著(P> 0. 05)。在细胞学检查方面,亚急性甲状腺炎典型的病理表现为中性粒细胞浸润、多核巨细胞浸润、肉芽肿性炎,而桥本氏甲状腺炎则以淋巴细胞浸润为主要表现。当症状不典型的亚急性甲状腺炎病例的B超表现及实验室检查无法与桥本氏甲状腺炎相鉴别、或亚急性甲状腺炎与桥本氏甲状腺炎合并存在时,可以通过液基细胞学检查辅助进行鉴别诊断,从而提高疾病诊断率,有效指导治疗,防止出现误诊及不良预后。
To explore the value of puncture liquid-based cytology of thyroid gland in the differential diagnosis of subacute thyroiditis and Hashimoto’ s thyroiditis,thyroid function,related antibody tests and thyroid puncture-based cytology were performed in37 patients with newly diagnosed subacute thyroiditis and 34 patients diagnosed with Hashimoto’s thyroiditis. There were significant differences in FT3,FT4,TSH,TGAb,and TPOAb between thyroid function and thyroid-related antibody markers in both groups( P < 0. 01),while TRAb was not significantly different( P > 0. 05). In cytology,the typical pathological manifestations of subacute thyroiditis are neutrophil infiltration,multinucleated giant cell infiltration,and granulomatous inflammation,while Hashimoto’ s thyroiditis is characterized by lymphocyte infiltration. When the ultrasonographic and laboratory tests of subacute thyroiditis with symptoms are not distinguishable from Hashimoto’ s thyroiditis,or subacute thyroiditis and Hashimoto’ s thyroiditis are combined,liquid-based cytology can be used. It assists in differential diagnosis improving the diagnosis rate of diseases,effectively guiding treatment,preventing misdiagnosis and poor prognosis.
