阶梯式进食安全管理对老年痴呆合并吞咽障碍效果观察 被引量:1

摘要 目的:观察阶梯式进食安全管理对老年痴呆合并吞咽障碍的效果。方法:选取笔者所在医院2018年1-12月收治的老年痴呆合并吞咽障碍患者66例,根据随机数字表法将其分为对照组及试验组,各33例。对照组采取常规进食管理,而试验组采取阶梯式进食安全管理。比较两组患者管理前后营养状况、吞咽障碍筛查评分(Burke)、免疫指标水平及管理过程中不良事件的发生率。结果:管理后两组患者BMI、Hb、ALB、免疫指标水平均明显升高,Burke评分明显降低,且试验组均明显优于对照组,差异有统计学意义(P<0.05)。试验组患者管理过程中不良事件发生率为3.03%,对照组不良事件发生率为24.24%,两组比较差异有统计学意义(P<0.05)。结论:采取阶梯式进食安全管理干预,可明显提高老年患者体质指数、血红蛋白及血清蛋白水平,减轻吞咽障碍,减少管理过程中不良事件的发生,提高机体免疫功能指标水平,效果理想。 Objective: To observe the effect of ladder type feeding safety management on senile dementia with dysphagia. Method: A total of 66 patients with senile dementia with dysphagia admitted in our hospital from January to December 2018 were selected. They were divided into the control group and the experimental group according to the method of random number table, 33 cases in each group. The control group received routine feeding management, while the experimental group adopted ladder type feeding safety management. The nutritional status, the screening score of dysphagia(Burke), the level of immune index and the incidence of adverse events were compared between the two groups before and after management. Result: After management, BMI, Hb, ALB and immune indexes levels of the two groups were significantly increased, and Burke score was significantly decreased, and the indexes in the experimental group were significantly better than the those of control group, the differences were statistically significant(P<0.05). The incidence of adverse events was 3.03% in the experimental group, and 24.24% in the control group. The difference between the two groups was statistically significant(P<0.05). Conclusion: ladder type feeding safety management can significantly improve the body mass index, hemoglobin and serum protein levels of elderly patients, reduce the degree of dysphagia, reduce the occurrence of adverse events in the management process, and improve the level of immune function indicators, with ideal effect.
