摘要
目的探讨二维斑点追踪成像技术对早期肺源性心脏病(简称肺心病)患者左心室功能的评估价值。方法选取2016年11月-2018年5月在绵阳市第三人民医院就诊的早期肺心病患者42例作为观察组,同期体检健康者42例作为对照组。比较两组受试者常规超声心动图检查指标及二维斑点追踪成像技术参数,二维斑点追踪成像技术参数与早期肺心病患者左心室射血分数的相关性分析采用Pearson相关分析。结果(1)观察组患者左心房内径、左心室舒张末期内径、左心室收缩末期内径长于对照组,左心室短轴缩短率、左心室射血分数低于对照组(P<0.05)。(2)观察组患者左心室长轴整体收缩期最大峰值应变、左心室轴向整体收缩期最大峰值应变、心脏整体收缩期应变率高于对照组,左心室整体扭转度、舒张早期应变率、舒张晚期应变率低于对照组(P<0.05)。(3)Pearson相关分析结果显示,左心室长轴整体收缩期最大峰值应变(r=-0.658)、左心室轴向整体收缩期最大峰值应变(r=-0.551)、心脏整体收缩期应变率(r=-0.564)与早期肺心病患者左心室射血分数呈负相关(P<0.05),而左心室整体扭转度(r=0.642)、舒张早期应变率(r=0.617)、舒张晚期应变率(r=0.526)与早期肺心病患者左心室射血分数呈正相关(P<0.05)。结论二维斑点追踪成像技术参数可有效反映早期肺心病患者左心室功能变化,适用于该类患者左心室功能的评估。
Objective To investigate the evaluative value of two-dimensional speckle tracking imaging on left ventricular function in patients with early pulmonary heart disease.Methods A total of 42 patients with early pulmonary heart disease were selected as observation group in the Third People’s Hospital of Mianyang from November 2016 to May 2018,and42 healthy people admitted to this hospital for physical examination were selected as control group.Routine echocardiography examination results and parameters of two-dimensional speckle tracking imaging were compared between the two groups;Pearson correlation analysis was used to analyze the correlations of parameters of two-dimensional speckle tracking imaging with LVEF in patients with early pulmonary heart disease.Results(1)LAD,LVEDD and LVESD in observation group were statistically significantly longer than those in control group,while LVFS and LVEF in observation group were statistically significantly lower than those in control group.(2)GLS,GCS and GSRs in observation group were statistically significantly higher than those in control group,while ROT,GSRe and GSRa in observation group were statistically significantly lower than those in control group(P<0.05).(3)Pearson correlation analysis results showed that,GLS(r=-0.658),GCS(r=-0.551)and GSRs(r=-0.564)was negatively correlated with LVEF in patients with early pulmonary heart disease,respectively(P<0.05),while ROT(r=0.642),GSRe(r=0.617)and GSRa(r=0.526)was positively correlated with the LVEF,respectively(P<0.05).Conclusion Parameters of two-dimensional speckle tracking imaging can effectively reflect the change of left ventricular function in patients with early pulmonary heart disease,which is applicable to evaluate the left ventricular function.
