摘要
目的分析高龄孕妇产前羊水诊断结果,探讨羊水穿刺术围术期护理配合对策。方法回顾性分析2015年3月-2019年7月期间于本院行羊水穿刺术的496例高龄孕妇的羊水诊断结果及妊娠结局,探讨围术期实施综合护理的效果。结果 496例高龄孕妇均穿刺成功,其中一次穿刺成功492例(99.19%),一次培养成功492例(99.19%),两次穿刺者在二周后均进行重新穿刺培养成功,均未发生流产及其他羊水穿刺术并发症。报告染色体异常119例,异常检出率(23.99%),其中数目异常8例,结构异常4例,嵌合体3例,性染色体异常17例,21-三体综合征67例,18-三体综合征17例,13-三体综合征3例。经综合护理后,496例高龄孕妇的平均焦虑评分、抑郁评分显著低于护理前(P <0.05);问卷调查孕妇对护理的满意度,其中406例非常满意,80例满意,10例不满意,护理满意度为97.98%。结论高龄孕妇染色体异常检出率高,产前及时进行羊水穿刺术筛查与诊断,有利于减少染色体异常出生率,在围术期应用综合护理能够明显减轻高龄孕妇的焦虑、抑郁程度,改善其心理状态,提高高龄孕妇对护理的满意度。
Objective To analyze the prenatal amniotic fluid diagnosis results of elderly pregnant women and explore the perioperative nursing cooperation strategies of amniocentesis. Methods The amniotic fluid diagnosis and pregnancy outcome of 496 elderly pregnant women who underwent amniocentesis in our hospital from March 2015 to July 2019 were analyzed retrospectively, and the effect of comprehensive nursing during perioperative period was discussed. Results 496 elderly pregnant women were successfully punctured, of which 492(99.19%) were successfully punctured once and 492(99.19%) were successfully cultured once. The two punctures were successfully re punctured and cultured two weeks later, and no abortion or other complications of amniocentesis occurred. 119 cases of chromosomal abnormality were reported. The detection rate of chromosomal abnormality was 23.99%, including 8 cases of numerical abnormality, 4 cases of structural abnormality, 3 cases of mosaicism, 17 cases of sexual chromosomal abnormality, 67 cases of 21 trisomy syndrome, 17 cases of 18 trisomy syndrome and 3 cases of 13 trisomy syndrome. After comprehensive nursing, the average anxiety score and depression score of 496 elderly pregnant women were significantly lower than those before nursing(P<0.05);questionnaire survey showed that 406 pregnant women were very satisfied, 80 were satisfied, 10 were not satisfied, the satisfaction rate of nursing was 97.98%. Conclusion Chromosome abnormality detection rate of elderly pregnant women is high. Prenatal amniocentesis screening and diagnosis is helpful to reduce the birth rate of chromosome abnormality. The application of comprehensive nursing during perioperative period can significantly alleviate the anxiety and depression of elderly pregnant women, improve their psychological state and improve their satisfaction with nursing.
