摘要
目的探讨联合检测同型半胱氨酸(homocysteine,Hcy)与促甲状腺激素(thyroid stimulating hormone,TSH)在甲状腺功能减退症诊断中的应用价值。方法本研究选取2015年1月-2018年12月在本院就诊的150例原发性甲状腺功能减退症患者为甲减组,同期选取本院体检中心体检的150例健康人群作为对照组,对两组Hcy、游离三碘甲状腺原氨酸(free triiodothyronine,FT3)、游离甲状腺素(free thyroxine,FT4)、TSH水平及各相关性进行比较。结果甲减组FT3、FT4水平显著低于对照组(P<0.05),Hcy、TSH水平显著高于对照组(P<0.05);原发性甲减患者血清Hcy与FT3呈显著负相关(r=-0.41,P<0.05),血清Hcy与FT4呈显著负相关(r=-0.32,P<0.05),血清Hcy与TSH水平呈显著正相关(r=0.62,P<0.05)。结论甲状腺功能减退症患者Hcy、TSH均呈高表达,且血清Hcy水平与FT3、FT4及TSH存在相关性,可将Hcy检测作为甲状腺功能减退症诊断的一项参考指标。
Objective To investigate the value of combined detection of homocysteine(Hcy) and thyrotropin(TSH) in the diagnosis of hypothyroidism. Methods 150 patients with primary hypothyroidism who were treated in our hospital from January 2015 to December 2018 were selected as group A, and 150 healthy people were selected as control group in the same period. The specificity, sensitivity and accuracy of Hcy, FT3, FT4 and TSH were compared. Results The levels of FT3 and FT4 in A subtraction group were significantly lower than those in control group(P<0.05), Hcy, TSH levels were significantly higher than those of the control group(P<0.05). There was a significant negative correlation between serum Hcy and FT3 in patients with primary hypothyroidism(r =-0.41, P<0. 05), a significant negative correlation between serum Hcy and FT4(r =-0.32, P< 0. 05), and a significant positive correlation between serum Hcy and TSH(r = 0. 62, P<0.05). Conclusion Hcy and TSH were expressed in patients with hypothyroidism, The level of serum Hcy was correlated with FT3, FT4 and TSH. Hcy detection can be used as a reference index for the diagnosis of hypothyroidism.
