摘要
目的评价北京市丰台区成人居民碘营养状况,探讨尿碘的影响因素。方法于2018年1—12月采用分层整群随机抽样方法抽取18~50岁成年男性、育龄妇女和孕妇各200名,收集检测研究对象的随机尿样并进行问卷调查,根据尿碘值将研究对象的碘营养状况分为碘缺乏、碘适宜、碘超适宜量、碘过量4个水平,使用有序多因素logistic回归模型进行多因素分析。结果590名调查对象完成调查,全部调查对象尿碘中位数(四分位数)[M(P25~P75)]为162.7(107.5~248.2)μg/L,其中成年男性、育龄妇女、孕妇尿碘M(P25~P75)分别为161.1(111.6~213.0)、198.2(107.1~288.7)、150.2(104.1~231.4)μg/L。总人群、成年男性、育龄妇女、孕妇碘缺乏比例分别为30.5%、19.5%、23.0%、50.0%,相应碘过量比例分别为12.7%、9.0%、23.0%、5.8%。多因素分析显示,年龄、文化程度与尿碘水平呈负相关,碘知识掌握情况、碘盐食用情况与尿碘水平呈正相关。不同人群的尿碘影响因素略有差异,育龄妇女组所有变量差异均无统计学意义(均P>0.05)。结论北京市丰台区成人整体处于碘适宜水平,但孕妇人群碘缺乏比例较高。相关部门应加强监测,持续评估丰台区居民的碘营养状况,关注孕妇等重点人群的碘营养情况,针对尿碘影响因素采取干预措施,指导居民维持适宜的碘营养水平。
Objective To estimate the status of iodine nutrition among adult residents in Fengtai District of Beijing,explore the influencing factors of urinary iodine.Methods A number of 600 participants were sampled with stratified cluster random method in 2018,including 200 men aged 18-50 years,200 women of childbearing age and 200 pregnant women.The random urinary iodine concentration was measured and the questionnaire was carried out for every participant from January-December 2018.According to the urinary iodine concentration,the status of iodine nutrition was divided into four levels:iodine deficiency,iodine appropriate,iodine overdose and excessive iodine.The ordered multivariate Logistic regression model was used for associated factors of urinary iodine levels.Results A total of 590 participants completed the investigation.The media M(P25-P75)of urinary iodine concentration of all subjects was 162.7(107.5-248.2)μg/L.The M(P25-P75)of urinary iodine concentration for men aged 18-50 years,women of childbearing age,pregnant women was 161.1(111.6-213.0),198.2(107.1-288.7)and 150.2(104.1-231.4)μg/L,respectively.The percent of all subjects,men,women of childbearing age and pregnant women at iodine deficiency was 30.5%,19.5%,23.0% and 50.0%,respectively,while the percent of excessive iodine was 12.7%,9.0%,23.0%,5.8%,respectively.The multivariate regression analysis showed that urinary iodine level was negatively correlated with age and education level,while there was a positive correlation between the level of urinary iodine and the knowledge of iodine and the consumption of iodized salt.There were slightly differences in influencing factors of urinary iodine among different population.There was no statistical significance in all variables of women of childbearing age group.Conclusion The adult residents are at proper iodine nutritional level in Fengtai District of Beijing,but the percent of iodine insufficiency is obviously high among pregnant women.Relevant departments should strengthen monitoring,continuously evaluate the iodine nutrition status of residents in Fengtai District,pay attention to the iodine nutrition status of pregnant women and other key groups,and take intervention measures to guide residents to maintain appropriate iodine nutrition level according to the influencing factors of urine iodine.
