摘要
目的观察狼疮性肾炎(LN)患者血清miR-4532、miR-2861表达变化,并探讨其临床意义。方法选择80例LN患者,其中活动期组(SLEDAI评分≥10分)42例、稳定期组(SLEDAI评分<10分)38例,另选取同期健康人员40例作为对照组。采用逆转录-聚合酶链反应技术检测血清miR-4532、miR-2861表达,采用Pearson相关性分析血清miR-4532、miR-2861表达与IL-6、IL-18、肾小球滤过率、24 h尿蛋白定量、SLEDAI评分、补体C3、补体C4的关系,通过受试者工作特征(ROC)曲线分析血清miR-4532、miR-2861表达对LN的诊断效能。结果活动期组、稳定期组血清miR-4532、miR-2861均较对照组低(P均<0.05),且活动期组较稳定期组低(P均<0.05)。LN活动期组患者血清miR-4532、miR-2861表达与血清IL-6、IL-18、24 h尿蛋白定量以及SLEDAI评分均呈负相关,而与肾小球滤过率呈正相关(P均<0.05)。血清miR-4532诊断LN的ROC曲线下面积为0.732(95%CI:0.651~0.834),最佳阈值为7.58,敏感度为81.14%,特异度为64.23%;miR-2861诊断LN的ROC曲线下面积为0.741(95%CI:0.623~0.802),最佳阈值为8.90,敏感度为78.35%,特异度为66.82%。结论miR-4532、miR-2861在LN患者血清中表达降低,其表达变化与患者病情进展及炎症反应相关,对LN临床诊断和病情评估具有重要的应用价值。
Objective To observe the expression changes of miR-4532 and miR-2861 in patients with lupus nephritis(LN),and to explore their clinical significance.Methods Eighty patients with LN were selected,including 42 patients in the active group(SLEDAI score≥10),38 patients in the stable group(SLEDAI score<10);meanwhile,40 healthy people in the same period were taken as the control group.Reverse transcription polymerase chain reaction(RT-PCR)was used to detect the expression of miR-4532 and miR-2861.Pearson correlation analysis was used to analyze the relationship between the expression of miR-4532 and miR-2861 and IL-6,IL-18,glomerular filtration rate,24-h urine protein quantity,SLEDAI score,complement C3,and complement C4.The diagnostic efficacy of miR-4532 and miR-2861 expression on LN was analyzed by ROC curve.Results The serum levels of miR-4532 and miR-2861 in the active group and the stable group were lower than those in the control group(both P<0.05),and those in the active group were lower than those in the stable group(both P<0.05).The expression of miR-4532 and miR-2861 in the active group was negatively correlated with the levels of serum IL-6,IL-18,24 h urinary protein and SLEDAI score,but was positively correlated with glomerular filtration rate(all P<0.05).The area under ROC curve of serum miR-4532 was 0.732(95%CI:0.651-0.834),the best threshold was 7.58,the sensitivity was 81.14%,and the specificity was 64.23%;the area under ROC curve of miR-2861 was 0.741(95%CI:0.623-0.802),the best threshold was 8.90,the sensitivity was 78.35%,and the specificity was 66.82%.Conclusion The miR-4532 and miR-2861 expression levels in serum of LN patients decrease,and are related to the progress of patients'condition and inflammatory reaction,which have important application value for the clinical diagnosis and condition evaluation of LN.
