鼻咽癌3.0T MRI动态增强参数与乏氧诱导因子-1α表达的相关性分析 被引量:4

摘要 目的探讨鼻咽癌3.0T MRI动态增强参数与乏氧诱导因子表达的相关性。方法对17例初诊鼻咽癌患者进行常规MRI扫描后行动态增强扫描,工作站后处理获取相对强化率、最大相对强化率、强化峰值、达峰时间、曲线下面积等参动态增强参数值;根据动态增强感兴趣区指导活检,组织标本进行HIF-1α检测。结果相对强化率、最大相对强化率、强化峰值、达峰时间、曲线下面积等数值分别为(132.51±12.23)%、(149.89±17.81)%、(1199.49±383.53)、(134.26±61.18)s、(298820.54±112728),17例鼻咽癌活检标本均呈HIF-1α阳性。相对强化率、最大相对强化率、强化峰值与HIF-1α表达呈正相关,曲线下面积与HIF-1α呈负相关(γ=-0.62,P=0.03),达峰时间与HIF-1α无相关性(P>0.05)。结论3.0T MRI动态增强参数值与鼻咽癌HIF-1α存在相关性,能够一定程度反映肿瘤的乏氧状态,为临床治疗提供重要依据。 Objective To investigate the correlation between the 3.0T MRI dynamic enhancement parameters of nasopharyngeal carcinoma and the expression of hypoxia-inducible factor.Methods 17 patients with newly diagnosed nasopharyngeal carcinoma underwent dynamic enhanced scanning after routine MRI scan.The relative enhancement rate,maximum relative enhancement rate,peak intensity,peak time,and area under the curve were obtained after workstation post-processing.Dynamically enhanced parameter values;guided biopsy according to the dynamic enhanced region of interest,tissue specimens and HIF-1αdetection,HIF-1αexpression level data recorded as 0-4 points.The non-parametric Spearman correlation test wasperformed on the dynamic enhancementparameter values and HIF-1αexpression levels.Results The relative enhancement rate,maximum relative enhancement rate,enhanced peak value,peak time,and area under the curve were(132.51±12.23)%,(149.89±17.81)%,(1194.49±383.53),(134.26±61.18)s and(298820.54±112728),respectively.17 cases of nasopharyngeal carcinoma biopsy specimens were positive for HIF-1α.The relative enhancement rate,maximum relative enhancement rate,and enhanced peak were positively correlated with HIF-1αexpression(γ=0.71,P=0.04;γ=0.84,P=0.03;γ=0.61,P=0.04),area under the curve and HIF-1αwas negatively correlated(γ=-0.62,P=0.03)and there was no correlation between peak time and HIF-1α(P>0.05).Conclusion The 3.0 T MRI dynamic enhancement parameter value is associated with HIF-1αin nasopharyngeal carcinoma,which can reflect the hypoxic status of the tumor to a certain extent andprovide an important basis for clinical treatment.
