早期脊椎结核疾病炎性水肿的影像学诊断价值评估 被引量:1

摘要 目的:探讨不同影像检查技术在早期脊椎结核炎性水肿诊断中的应用价值。方法:回顾性分析医院接治的63例脊椎结核疑似患者,均行X射线、CT和MRI检查,按照检查方式的不同,将患者的影像诊断结果分别设为X射线组、CT组和MRI组,每组影像资料均为63例。对比不同影像学检查方式下脊椎结核疾病和早期脊椎结核炎性水肿的诊断真阳性率、真阴性率和准确率。结果:在脊椎结核疾病的诊断方面,CT组和MRI组的真阳性率、真阴性率及准确率结果优于X射线组,差异均具有统计学意义(x2真阳性率=14.592、x2=23.251,x2真阴性率=5.250、x2=7.337,x2准确率=19.722、x2=30.406;P<0.05),CT组和MRI组的差异无统计学意义;在早期脊椎结核炎性水肿的诊断方面,CT组和MRI组的真阳性率、真阴性率及准确率结果优于X射线组,差异均具有统计学意义(x2真阳性率=10.096、x2=24.436,x2真阴性率=9.894、x2=28.233,x2准确率=18.402、x2=50.428;P<0.05),且MRI组的所有指标均优于CT组,其差异有统计学意义(x2=4.725、x2=7.265、x2=11.649;P<0.05)。结论:CT和MRI检查技术在脊椎结核疾病诊断方面具有一定优势,但在早期脊椎结核炎性水肿的鉴别诊断方面,MRI是一种最准确的影像检查技术。 Objective:To explore the application value of different imaging examination techniques in diagnosis of inflammatory edema of spinal tuberculosis in early stage.Methods:63 suspected patients with spinal tuberculosis admitted to the hospital were retrospective analyzed.All of these patients underwent X-ray examination,CT examination and MRI examination.According to the different examination method,namely general X-ray,CT and MR,the imaging diagnosis results of these patients were classified into the group of X-ray,the group of CT and the group of MRI,respectively.The imaging data of each group included 63 cases.For spinal tuberculosis and inflammatory edema of spinal tuberculosis in early stage,the true positive rate,true negative rate and accuracy rate of the diagnosis of different imaging examination methods were further compared.Results:In the aspect of diagnosing spinal tuberculosis,the true positive rates,true negative rates and accuracy rates of the group of CT and the group of MRI were significantly better than those of the group of X-ray(x2true positive rate=14.592,x2=23.251,x2true negative rate=5.250,x2=7.337,x2accuracy rate=19.722,x2=30.406,P<0.05).There was no significant differences about these indicators between the group of CT and of MRI.In diagnosing inflammatory edema of spinal tuberculosis in early stage,the results of the group of CT and the group of MRI were significantly better than those of the group of X-ray,respectively(x2true positive rate=10.096,x2=24.436,x2true negative rate=9.894,x2=28.233,x2accuracy rate=18.402,x2=50.428,P<0.05).All the indicators in the group of MRI were significantly better than those in the group of CT(x2=4.725,x2=7.265,x2=11.649,P<0.05).Conclusions:CT and MRI have certain advantages in the diagnosis of spinal tuberculosis,and then MRI is the most accurate imaging examination technique for inflammatory edema of spinal tuberculosis in early stage.
