摘要
目的:初步探究临床常见结肠病变患者Th1/Th17/Th2细胞相关细胞因子在外周血免疫微环境中的表达情况。方法:采用酶联免疫吸附法(ELISA)、实时荧光定量PCR、流式细胞术检测45例健康体检者、60例结肠息肉患者、51例溃疡性结肠炎患者外周血IL-4、IL-17、IFN-γ的表达量,分析与中性/淋巴细胞比值的相关性及性别影响。结果:结肠息肉组IL-4的胞外表达低于正常组;而溃疡性结肠炎组在细胞内外表达高于正常组,两病变组IL-4的表达量与外周血中性/淋巴相比均无显著相关性;和健康人群相比,结肠息肉和溃疡性结肠炎组IL-17在细胞内外表达水平为相反的变化趋势,与外周血中性/淋巴相比分别呈现负相关和正相关,其中溃疡性结肠炎的IL-17胞内表达显著升高,男性患者IL-17胞内表达高于女性患者;肠病患者IFN-γ胞外表达显著高于正常人群,均与外周血中性/淋巴比存在正相关。结论:结肠息肉患者外周血中Th1细胞胞外活性上调,正反馈调节Th2的胞内活性;溃疡性结肠炎患者外周血Th1/Th2细胞胞外活性均升高,抑制Th1细胞分化,促进Th2/Th17细胞分化;Th17细胞在两种肠病患者中具有双向调节功能,与外周血中性粒细胞数量和性别有一定关联。
Objective:Investigate the expression of Th1/Th17/Th12 cell subsets associated cytokines in peripheral blood immune microenvironment of patients with common colonic diseases.Methods:Detected the expression of IL-4,IL-17,IFN-γin peripheral blood of 45 healthy people,60 colonic polyps patients and 51 ulcerative colitis patients by enzyme-linked immunosorbent assay(ELISA),flow cytometry and real-time fluorescence quantitative PCR.Analyze the correlation between the expression of cytokines and the neutrophil/lymphoid ratio and sex factor.Results:The extracellular IL-4 in colonic polyp group was lower than that in normal group,while the expression of extracellular and intracellular IL-4 in ulcerative colitis group was higher than normal group.There was no significant correlation between the expression of IL-4 and peripheral blood neutrophil/lymphocyte ratio in patients.Compared with healthy people,the expression level of IL-17 in colon polyps and ulcerative colonitis group showed opposite change trend,and showed negative and positive correlation with peripheral blood neutrophil/lymphocyte ratio,respectively.The intracellular expression of IL-17 in ulcerative colitis was significantly increased,and the expression of IL-17 in male patients was higher than that in female patients.Compared with the normal group,the expression of IFN-γin patients with intestinal disease was significantly up-regulated,in the meanwhile,there was a positive correlation between the expression of IFN-γand neutrophil/lymphoid ratio in peripheral blood.Conclusion:The extracellular activity of Th1 cells was up-regulated in peripheral blood of patients with colon polyp,and the extracellular activity of Th1/Th2 cells in peripheral blood of patients with ulcerative colitis was increased,which inhibited the differentiation of Th1 cells and promoted the differentiation of Th2/Th17 cells,and positively regulated the intracellular activity of Th1/Th2 cells in the peripheral blood of patients with ulcerative colitis.Th17 cells have two-way regulation function in two kinds of colonic diseases,which is related to the number and sex of neutrophils in peripheral blood.
