β-catenin和MMP-3在骨性关节炎软骨组织中的表达及临床意义 被引量:12

Expression and clinical significance of β-catenin and MMP-3 in cartilage tissue of osteoarthritis

摘要 目的检测不同时期膝关节骨性关节炎软骨中β-连环蛋白(β-catenin)和基质金属蛋白酶-3(MMP-3)的表达。探讨这两种蛋白在骨性关节炎患者疾病进展中的作用及临床意义。方法OA组:取自因骨性关节炎而行人工膝关节置换术(TKA)或关节镜诊治术的患者,共计70例;NOA组:取自创伤后导致的截肢或部分软骨脱落使用关节镜可取出的患者,共计13例。根据WOMAC骨关节炎指数,将70例OA患者分为轻度OA组18例,中度OA组22例,重度OA组30例。免疫组化法对β-连环蛋白和MMP-3的各组阳性细胞进行染色,根据两种蛋白的表达情况,用软件SPSS 22.0统计分析出两种蛋白与OA发展的相互关系。结果β-连环蛋白免疫组化染色结果:NOA组有2例阳性,阳性率为15.38%;OA组患者关节软骨中,轻度OA组10例阳性,阳性率55.56%;中度OA组17例,阳性率77.27%,重度OA组27例,阳性率90.00%。MMP-3免疫组化染色结果:NOA组有1例阳性,阳性率为7.69%;OA组患者关节软骨中,轻度OA组9例阳性,阳性率50.00%;中度OA组14例阳性,阳性率63.64%,重度OA组25例阳性,阳性率83.33%。卡方检验结果示β-连环蛋白与MMP-3在骨性关节炎组阳性表达率明显高于非骨关节炎组(X^2=19.053/16.803,P<0.001)。采用Spearman相关分析法结果提示:软骨组织中β-连环蛋白与MMP-3的表达水平随着WOMAC骨关节炎指数升高而增强,呈显著正相关(r=0.753/0.787,P<0.001);β-连环蛋白与MMP-3的表达水平随着Mankin评分的升高而增强,呈显著正相关(r=0.840/0.774,P<0.001),在OA关节软骨中β-catenin与MMP-3的表达强度之间存在显著正相关(r=0.891,P<0.001)。结论(1)关节软骨中β-catenin和MMP-3的表达较非骨关节炎组显著增强,且随着OA不断加重这两种蛋白的表达强度不断增强,这两种蛋白可作为OA早期诊断的依据。(2)β-catenin、MMP-3参与了OA发生发展过程中软骨的破坏。(3)OA软骨组织中β-catenin、MMP-3表达呈显著正相关,两者对OA的发生可能存在协同作用。 Objective To detect the expression ofβ-catenin and matrix metalloproteinase-3(MMP-3)in cartilage of knee osteoarthritis at different stages.To explore their role and clinical significance in the progression of disease in patients with osteoarthritis.Methods OA group:70 patients who underwent artificial knee arthroplasty(TKA)or arthroscopic diagnosis and treatment for osteoarthritis;NOA group:used for amputation or partial cartilage loss after trauma A total of 13 patients with a mirror can be removed.According to the WOMAC Osteoarthritis Index,70 patients with OA were divided into 18 patients with mild OA,22 patients with moderate OA,and 30 patients with severe OA.The positive cells ofβ-catenin and MMP-3 were stained by immunohistochemistry.According to the expression of the two proteins,the relationship between the two proteins and OA was analyzed by software SPSS 22.0.Results Immunohistochemical staining results ofβ-catenin:2 cases were positive in NOA group,the positive rate was 15.38%;in group OA group,10 cases were positive in mild OA group,positive rate was 55.56%;moderate OA group 17 For example,the positive rate was 77.27%,and the severe OA group was 27 cases,and the positive rate was 90.00%.MMP-3 immunohistochemical staining results:1 case was positive in the NOA group,the positive rate was 7.69%;in the articular cartilage of the OA group,9 cases were positive in the mild OA group,the positive rate was 50.00%;14 cases in the moderate OA group were positive.The positive rate was 63.64%,and 25 cases were positive in the severe OA group,with a positive rate of 83.33%.Chi-square test showed that the positive expression rate ofβ-catenin and MMP-3 in the osteoarthritis group was significantly higher than that in the non-osteoarthritis group(X ^2=19.053/16.803,P<0.001).The results of Spearman correlation analysis indicated that the expression levels ofβ-catenin and MMP-3 in cartilage tissue increased with the increase of WOMAC osteoarthritis index,showing a significant positive correlation(r=0.753/0.787,P<0.001).The expression levels ofβ-catenin and MMP-3 increased with the increase of Mankin score,showing a significant positive correlation(r=0.840/0.774,P<0.001),andβ-catenin and MMP-3 in OA articular cartilage.There was a significant positive correlation between expression intensity(r=0.891,P<0.001).Conclusion(1)The expression ofβ-catenin and MMP-3 in articular cartilage is significantly enhanced compared with non-osteoarthritis group,and the expression intensity of these two proteins is increasing with the increasing OA.These two proteins can be used as early OA.The basis for diagnosis.(2)β-catenin and MMP-3 are involved in the destruction of cartilage during the development of OA.(3)There was a significant positive correlation between the expression ofβ-catenin and MMP-3 in OA cartilage tissue,and there may be a synergistic effect on the occurrence of OA.
