摘要
目的探究老年2型糖尿病(type 2 diabetes mellitus,T2DM)下肢血管病变患者的临床特征,评估超声检查在T2DM下肢血管病变诊断中的价值,为该病的诊治提供依据。方法选取2018年9月-2019年10月海口市人民医院收治的102例T2DM患者的临床资料进行回顾性分析,根据彩色多普勒超声诊断结果分为观察组(患有下肢血管病变患者)和对照组(无下肢血管病变患者),每组51例。评估并比较2组的血压、血脂、下肢血管病变彩超检查结果、下肢血管内膜厚度以及血流参数等。结果 2组下肢血管病变彩超检查结果比较,观察组下肢血管病变易损斑块数量和IMP值均明显高于对照组(P<0.05);观察组IMT值高于对照组(P<0.05),而PSV和RI值均低于对照组(P<0.05);治疗后观察组SBP、DBP、TG和TC水平均低于对照组(P<0.05)。结论对2型糖尿病下肢血管病变可采取彩色多普勒超声进行诊断,快速和准确,操作简单,误诊率低,具有一定的临床应用价值。
Objective To explore the clinical features of lower extremity vascular disease in the elderly with type 2 diabetes mellitus(T2DM) and to evaluate the diagnostic value of ultrasonography.Methods A retrospective analysis was made to the clinical data of 102 cases with T2DM during the period from September,2018 to October,2019;the 102 cases were divided,based on the color Doppler ultrasound diagnosis,into 2 groups:observation group(cases with lower extremity vascular disease) and control group(cases with no lower extremity vascular disease),51 cases in each;the blood pressure,the blood lipid,the color Doppler examination of lower extremity vascular lesions,the intima thickness of lower extremity blood vessels and the blood flow parameters were evaluated and a comparative study was made between the 2 groups.Results The number of vulnerable plaques and the level of IMP of the cases in observation group were bigger and higher than those in control group(P<0.05);the IMT level was higher and the levels of PSV and RI were lower in observation group than those in control group(P<0.05);the levels of SBP,DBP,TG and TC in observation group were lower than those in control group after treatment(P<0.05).Conclusions Color Doppler ultrasound examination is quick and accurate in diagnosing T2DM lower extremity vascular lesions with simple operation and less misdiagnosis,it is of certain value of clinical application.
