摘要
目的:分析纤维支气管镜检查结合胸部CT对中央型肺癌的诊断价值。方法:纳入研究对象来自本院2017年2月-2019年2月收治的80例中央型肺癌患者,分别行纤维支气管镜检查、胸部CT检查、纤维支气管镜检查结合胸部CT检查,将穿刺或者术后病理检查结果作为金标准,计算三种检查方法的诊断符合率。结果:80例患者均经穿刺或者术后病理检查确诊,确诊率为100%。纤维支气管镜检查结合胸部CT检查诊断符合率97.50%显著高于纤维支气管镜检查75.00%、胸部CT检查77.50%,差异有统计学意义(P<0.05)。纤维支气管镜检出15例未分化癌,占25.00%;10例腺鳞癌,占16.67%;17例腺癌,占28.33%;12例小细胞肺癌,占20.00%;6例鳞癌,占10.00%。胸部CT检出14例未分化癌,占22.58%;12例腺鳞癌,占19.35%;11例腺癌,占17.74%;13例小细胞肺癌,占20.97%;12例鳞癌,占19.35%。纤维支气管镜结合胸部CT检出20例未分化癌,占25.64%;18例腺鳞癌,占23.08%;17例腺癌,占21.79%;16例小细胞肺癌,占20.51%;7例鳞癌,占8.97%。结论:纤维支气管镜检查结合胸部CT可显著提高中央型肺癌诊断准确率,为治疗方案的制定提供科学的参考依据,值得借鉴。
Objective:To analyze the diagnostic value of fiberoptic bronchoscopy combined with chest CT in central lung cancer.Method:A total of 80 patients with central lung cancer admitted to our hospital from February 2017 to February 2019 were examined by fiberoptic bronchoscopy,thoracic CT,fiberoptic bronchoscopy and thoracic CT respectively.The puncture or pathological examination results were taken as gold standard,and the diagnostic coincidence rates of the three methods were calculated.Result:They were diagnosed by puncture or pathological examination after operation,the diagnostic rate was 100%.The diagnostic coincidence rate of Fiberoptic Bronchoscopy combined with thoracic CT examination was 97.50%significantly higher than that of fiberoptic bronchoscopy(75.00%)and thoracic CT examination(77.50%),the difference was statistically significant(P<0.05).Fiberoptic bronchoscopy detected 15 cases(25.00%)with undifferentiated cancers,10 cases(16.67%)with adenosquamous carcinomas,17 cases(28.33%)with adenocarcinomas,12 cases(20.00%)with small cell lung cancer and 6 cases(10.00%)squamous cell carcinomas.Chest CT detected 14 cases(22.58%)with undifferentiated carcinomas,12 cases(19.35%)with adenosquamous carcinomas,11 cases(17.74%)with adenocarcinomas,13 cases(20.97%)with small cell lung cancers and 12 cases(19.35%)with squamous cell carcinomas.Fiberoptic Bronchoscopy Combined with chest CT detected 20 cases(25.64%)with undifferentiated cancers,18 cases(23.08%)with adenosquamous carcinomas,17 cases(21.79%)with adenocarcinomas,16 cases(20.51%)with small cell lung cancer and 7 cases(8.97%)with squamous cell carcinomas.Conclusion:Fiberoptic bronchoscopy combined with chest CT can significantly improve the diagnostic accuracy of central lung cancer,and provide a scientific reference for the formulation of treatment plan,which is worth learning.
