摘要
目的探讨甲状腺功能亢进患者血清胰岛素样生长因子1(IGF-1)与内脂素的变化及临床意义。方法选取2016年1月至2018年12月我科收治的甲状腺疾病患者235例,其中甲亢组150例、甲减组85例,并选取健康对照组60例,分别检测各组受试者的血清胰岛素样生长因子1、内脂素水平;比较各组间血清IGF-1、内脂素水平差异,并分析血清IGF-1、内脂素水平对甲状腺功能亢进的早期诊断预测价值。结果甲亢组血清IGF-1水平高于甲减组和对照组(P<0.05);甲亢组血清内脂素水平与甲减组、对照组相比,差异具有统计学意义(P<0.05)。甲亢组患者血清IGF-1、内脂素水平治疗前与治疗后相比,差异具有统计学意义(P<0.05)。多因素分析结果示:血清FT 3、FT 4、TT 3、TT 4、TPOAb、IGF-1、内脂素水平升高、血清TSH水平降低是甲状腺功能亢进症的危险因素。血清IGF-1曲线下面积为0.784,灵敏度和特异性分别为81.7%和78.4%;内脂素曲线下面积为0.820,灵敏度和特异性分别为86.9%和80.5%;血清IGF-1、内脂素联合检测曲线下面积为0.913,灵敏度和特异性分别为93.7%和89.8%。结论血清IGF-1与内脂素水平变化在甲状腺功能亢进症发生发展中起重要作用,可作为甲状腺功能亢进症临床辅助诊断的参考指标。
Objective To investigate the changes and clinical significances of serum insulin-like growth factor 1(IGF-1)and visfatin in patients with hyperthyroidism.Methods A total of 235 patients with thyroid disease admitted to our department from January,2016 to December,2018 were enrolled for the study,including 150 patients with hyperthyroidism,85 patients with hypothyroidism,and we also selected 60 healthy controls.Serum insulin samples were obtained from each group.Growth factor 1 and visfatin levels were measured among all subjects.We then compared serum IGF-1,visfatin levels between groups,conducted the analysis of serum IGF-1,visfatin levels for early diagnosis and prediction of hyperthyroidism.Results The serum IGF-1 levels in the hyperthyroidism group were higher than those in the hypothyroidism group and the control group(P<0.05).The serum visfatin levels detected by the hyperthyroidism group were significantly different from those of the hypothyroidism group and the control group(P<0.05).The serum levels of IGF-1 and visfatin in patients with hyperthyroidism were significantly different before and after the treatment(P<0.05).Multivariate analysis showed that serum FT 3,FT 4,TT 3,TT 4,TPOAb,IGF-1,elevated visfatin levels,and decreased serum TSH levels were risk factors for hyperthyroidism.The area under the curve for serum IGF-1 was 0.784,with the sensitivity and specificity 81.7%and 78.4%,respectively,while the area under the curve for visfatin was 0.820,with the sensitivity and specificity 86.9%and 80.5%,respectively.The area under the curve for combined detection was 0.913,with the sensitivity and specificity 93.7%and 89.8%.Conclusion The changes of serum IGF-1 and visfatin levels play an important role in the development of hyperthyroidism,which can be used as a reference for clinical diagnosis of hyperthyroidism.
