临床分离大肠埃希菌的耐药性与耐药基因研究

摘要 目的分析临床分离的大肠埃希菌的耐药情况和相关耐药基因存在状况。方法收集2016年1月至2017年7月上饶市人民医院临床分离的各类标本,采用全自动微生物鉴定分析仪对436株大肠埃希菌进行细菌鉴定与药敏分析,选取了30株多重耐药的大肠埃希菌进行TEM型β-内酰胺酶基因、aac3-Ⅱ和aac(6′)-Ⅰb型氨基糖苷类修饰酶基因、DHA和ACT-1型AmpC酶基因、qacEΔ1-sul1耐消毒剂和磺胺基因、catA,catB氯霉素酰基转移酶基因及cmlA外排泵基因的研究。结果30株大肠埃希菌对氨苄西啉耐药率达100.0%、对头孢唑林耐药率达76.6%(23/30)、头孢呋辛耐药率达80%(24/30)、头孢曲松为73.3%(22/30)、左氧氟沙星53.3%(16/30)、环丙沙星53.3%(16/30)、复方新诺明为90%(27/30),对阿米卡星的耐药率较低为3.3%,对呋喃妥因、美罗培南及亚胺培南的耐药率分别为0。TEM基因、qacEΔ1-sul1耐消毒剂和磺胺基因、aac(6′)-Ⅰb型氨基糖苷类修饰酶基因检测全为阳性。结论临床分离的大肠埃希菌存在严重耐药情况,耐药基因携带率高。应加强细菌耐药性的检测,指导临床合理应用抗生素。 Objective Analysis of drug resistance and researchs drug resistance-related genes ofEscherichia coli isolated from clinical isolates.Methods The samples of the clinical samples from shangrao January 2016 to July 2017 were separated and identified by the automatic microbiological identification analyzer.Select 30 strains drug resistance ofEscherichia coli so as to analyze TEM gene、qacEΔ1-sul1、catA,catB gene、cmlA gene.Results Thirty strains ofEscherichia coli on ampicillin resistance rate of 100%、76.6%(23/30)for cefazolin 80%(24/30)for ceftriaxone 73.3%(22/30)for levofloxacin ciprofloxacin 53.3%(16/30),Compound Sulfamethexazde 90%(27/30),resistance to amikacin was lower 3.3%,resistance rates to nitrofurantoin,meropenem and imipenem were 0.Conclusion Escherichia coli have serious drug resistance and high carrying rate of drug resistant genes in isoIated.We should strengthen the drug resistance of pathogens,guiding clinical rational use of antibiotics.
