摘要
目的:探索首发青年期广泛性焦虑障碍(GAD)患者的认知功能特征及其与下丘脑-垂体-肾上腺轴(HPA轴)外周血皮质醇和脑-垂体-甲状腺轴(HPT轴)甲状腺激素水平的相关性。方法:比较31例首发青年期GAD患者(GAD组)与20名健康对照者(对照组)在临床特征、MATRICS共识认知成套测验(MCCB)7个领域的认知功能得分及外周血皮质醇和甲状腺激素浓度的差异及其相关性。结果:GAD组在注意/警觉性、推理及问题解决和视觉学习领域得分低于对照组(P<0.05或P<0.01)。GAD组HAMA总分与皮质醇浓度呈正相关,与MCCB各分量表及总分呈负相关(P<0.05或P<0.01);病程与T4浓度呈负相关(P<0.05);TSH水平与注意/警觉性得分呈负相关(P<0.05)。结论:首发青年期GAD患者存在HPA轴、HPT轴活动的紊乱以及认知功能不同方面的受损。
Objective: To explore the characteristics of cognitive function and its correlation with peripheral blood cortisol from hypothalamus-pituitary-adrenal cortex axis and thyroid hormone levels from hypothalamus-pituitary-thyroid axis in first-episode adolescent patients with generalized anxiety disorder(GAD). Method:The differences and correlations in clinical features,7 cognitive function scores tested by MATRICS Consensus Cognitive Battery(MCCB),and concentrations of cortisol and thyroid hormone of peripheral blood were compared between 31 first-onset adolescent GAD patients and 20 healthy controls. Results:The scores of attention/alertness,reasoning,problem solving and visual learning in the GAD group were lower than those in the control group(P<0.05 or P<0.01).HAMA total score in GAD patients was positively correlated with cortisol concentration but negatively correlated with each subscale and total score of MCCB(P<0.05 or P<0.01).The course of GAD was negatively correlated with T4 concentration(P<0.05).TSH level was negatively correlated with attention/alertness score(P<0.05). Conclusion:Adolescent patients with first-onset GAD have disorders of HPA,HPT axis activity,and impairment of cognitive function.
