摘要
目的分析对处于急性期的外伤性颅内血肿患者进行血浆D-二聚体测定的作用。方法以2017年8月—2019年5月112例急性期外伤性颅内血肿患者为对象,视作观察组,另外选取100名同期健康受检者为对照组,均进行血浆D-二聚体检测,分析检测结果。结果观察组D-二聚体水平为(347.63±42.57)μg/L,高于对照组(113.95±25.46)μg/L,差异有统计学意义(t=26.862,P<0.05);轻中重度颅内血肿患者D-二聚体水平分别为(226.67±19.86)μg/L、(316.34±42.15)μg/L、(427.46±50.37)μg/L,差异有统计学意义(P<0.05);硬脑膜下血肿、脑内血肿患者D-二聚体水平高于硬脑膜外血肿患者,差异有统计学意义(P<0.05);恢复良好、轻度残疾、重度残疾、死亡患者D-二聚体水平分别为(230.78±16.39)μg/L、(298.76±25.37)μg/L、(367.42±43.25)μg/L、(438.69±52.42)μg/L,差异有统计学意义(P<0.05)。结论外伤性颅内血肿急性期血浆D-二聚体水平直接联系患者病情严重程度以及疾病预后,临床应该注重对血浆D-二聚体的动态测定,及时进行相应干预处理。
Objective To analyze the effect of plasma D-dimer on patients with traumatic intracranial hematoma in acute phase.Methods A total of 112 patients with acute traumatic intracranial hematoma from August 2017 to May 2019 were enrolled in the observation group.Another 100 healthy subjects were selected as the control group,and plasma D-dimerization was performed.Body detection,analysis of test results.Results The level of D dimer in the observation group was(347.63±42.57)μg/L,which was higher than that of the control group(113.95±25.46)μg/L,the difference was statistically significant(t=26.862,P<0.05).The patients with mild to moderate intracranial hematoma D II levels of the polymer were(226.67±19.86)μg/L,(316.34±42.15)μg/L,(427.46±50.37)μg/L,the difference was statistically significant(P<0.05);D subdimer in patients with subdural hematoma and intracerebral hematoma was higher than that of patients with epidural hematoma,the difference was statistically significant(P<0.05);the levels of D-dimer in patients with good recovery,mild disability,severe disability and death were(230.78±16.39)μg/L,(298.76±25.37)μg/L,(367.42±43.25)μg/L,(438.69±52.42)μg/L,the difference was statistically significant(P<0.05).Conclusion The level of plasma D-dimer in the acute stage of traumatic intracranial hematoma is directly related to the severity of the disease and the prognosis of the disease.The clinical measurement of plasma D-dimer should be paid attention to and the corresponding intervention should be carried out in time.
