连续性肾脏替代疗法治疗重症乳酸酸中毒的效果观察 被引量:2

Effects of continuous renal replacement therapy in patients with severe lactic acid poisoning

摘要 目的探讨连续性肾脏替代疗法(CRRT)在重症乳酸酸中毒患者中的应用效果.方法选取2015年12月至2018年12月在武警山西总队医院治疗的乳酸酸中毒患者260例,按照随机数字表法将其分为两组,每组130例.对照组采取常规对症治疗,观察组在常规对症治疗基础上采取CRRT治疗.比较两组患者的病死率及治疗前、治疗后24 h血生化指标变化情况.结果观察组患者病死率(3.85%,5/130)低于对照组(16.92%,22/130),P<0.05.治疗前,两组血生化指标比较差异未见统计学意义(P>0.05);治疗24 h后,观察组pH值、二氧化碳结合力水平高于对照组,血肌酐、血乳酸、血尿素氮水平低于对照组(P<0.05).结论对重症乳酸酸中毒患者应用CRRT治疗可降低其病死率,改善血生化指标,促进乳酸清除. Objective To observe the application effects of continuous renal replacement therapy(CRRT)on patients with severe lactic acid poisoning.Methods A total of 260 patients with lactic acid poisoning treated in Shanxi Armed Police Hospital from December 2015 to December 2018 were selected,and they were divided into two groups according to random number table method,with 130 cases in each group.The control group was treated by routine symptomatic treatment,while the observation group was treated by CRRT on the basis of routine symptomatic treatment.The mortality rate and changes of blood biocliemical indexes before and 24 hours after treatment were compared between the two groups.Results The mortality rate of observation group(3.85%,5/130)was significantly lower than that of control group(16.92%,22/130,)P<0.05.Before treatment,there was no significant difference in blood biochemical indexes between the two groups(P>0.05).The levels of pH and C02 combining power in observation group were significantly higher than those in control group 24 hours after treatment,and the levels of blood lactic acid and blood urea nitrogen were significantly lower than those in control group(P<0.05).Conclusions CRRT can reduce mortality rate,improve blood biochemical indexes and promote lactic acid clearance in patients with severe lactic acid poisoning.
