摘要 目的:探讨动态增强磁共振(magnetic resonance imaging,MRI)检查的时间-信号强度曲线(TIC)与扩散加权成像(DWI)联合应用在疑似乳腺病变的检查效果。方法:对2017年2月~2018年9月在我院乳腺外科治疗的51例女性患者进行MRI动态增强和DWI,分析TIC和表观扩散系数(ADC)值,比较两者联合与单一运用的敏感度和特异度差异。结果:以II型和III型诊断为恶性病灶,敏感度90.91%(30/33),特异度50.00%(9/18);以ADC值<1.20×10-3 mm 2/s判定为恶性病灶,敏感度84.85%(28/33)特异度55.56%(10/18);TIC联合DWI的两者综合判断,敏感度96.97%(32/33),特异性83.33%(15/18)。结论:动态增强MRI的TIC与DWI联合应用可以提高对乳腺癌诊断特异度,具有较高的诊断价值。 Objective:To investigate the effect of time-signal intensity curve(TIC)and diffusion-weighted imaging(DWI)combined with dynamic enhanced magnetic resonance imaging(MRI)on suspected breast lesions.Methods:MRI dynamic enhancement and DWI were performed in 51 female patients who underwent breast surgery in our hospital from February 2017 to September 2018.The TIC and apparent diffusion coefficient(ADC)values were analyzed to compare the sensitivity of the two combined with single use.Differences in degree and specificity.Results:Type II and type III were diagnosed as malignant lesions with sensitivity of 90.91%(30/33)and specificity of 50.00%(9/18).Malignant lesions were determined with ADC value<1.20×10-3 mm 2/s.Sensitivity 84.85%(28/33)specificity 55.56%(10/18);TIC combined with DWI,the comprehensive judgment,sensitivity 96.97%(32/33),specificity 83.33%(15/18).Conclusion:The combination of TIC and DWI with dynamic enhanced MRI can improve the diagnostic specificity of breast cancer and has a high diagnostic value.
