摘要
目的探讨以临床路径为指导的谵妄管理对老年股骨头置换术后患者的影响。方法选取我院2015年3月至2019年3月股骨头置换术后老年患者80例作为研究对象,随机将其分为对照组36例和观察组44例,对照组术后给予常规护理干预,观察组在对照组基础给予以临床路径为指导的谵妄护理管理。比较两组患者术后12,24,72 h谵妄发生率,比较两组简易智力状况检查量表(MMSE)评分及谵妄持续时间、每日总睡眠时间。结果观察组术后24,72 h谵妄发生率明显低于对照组(P <0. 05);观察组术后1,3,7 d的MMSE评分明显高于对照组(P <0.05);观察组谵妄持续时间短于对照组(P <0. 05),每日总睡眠时间明显长于对照组(P <0. 05)。结论以临床路径为指导的谵妄管理在股骨头置换术后老年患者中应用效果满意,能够有效降低谵妄发生率,促进患者康复。
Objective To explore the effect of applying delirium management guided by clinical pathway for elderly patients after femoral head replacement. Methods To select 80 cases of elderly patients after femoral head replacement in our hospital from March 2015 to March 2019 as the study objects.They were randomly divided into a control group for 36 cases and an observation group for 44 cases. The control group was given routine nursing intervention after surgery. The observation group was given delirium care management guided by the clinical pathway based on the control group. The incidence of delirium at 12,24,and 72 hours after surgery was compared between the two groups,as well as the mini-mental state examination( MMSE) score,delirium duration,and total daily sleep time were compared between the two groups. Results The incidence of delirium at 24 and 72 hours after surgery in the observation group was significantly lower than that in the control group( P < 0. 05). The MMSE scores on 1,3 and 7 days after surgery in the observation group were significantly higher than those in the control group( P < 0. 05). The delirium duration in the observation group was shorter than that in the control group( P < 0. 05). The total daily sleep time in the observation group was significantly longer than that in the control group( P < 0. 05). Conclusion The management of delirium guided by clinical pathways was satisfactory in elderly patients after femoral head replacement,which could effectively reduce the incidence of delirium and promote patient rehabilitation.
