潮气呼吸肺功能检测在婴幼儿喘息治疗效果评价中的应用 被引量:14

摘要 目的观察潮气呼吸肺功能检测在婴幼儿喘息治疗效果评价中的作用。方法 114例喘息患儿根据哮喘预测指数(API)分为API阴性组(64例)和API阳性组(50例),两均给予抗炎、抗过敏、舒张气道等规范治疗;另取50例同期儿童生长发育门诊体检的健康婴幼儿作对照(对照组)。各组均进行潮气呼吸肺功能检测,比较API阴性组、API阳性组治疗前后及对照组达峰时间比(TPTEF/TE)、达峰容积比(VPEF/VE)、每公斤潮气量(Vt/kg)、吸气时间/呼气时间(Ti/Te)、呼出25%潮气容积时的呼气流量(TEF75)、呼出50%潮气容积时的呼气流量(TEF50)、呼出75%潮气容积时的呼气流量(TEF25)。结果与同组治疗前比较,API阳性组和API阴性组治疗后TPTEF/TE、VPEF/VE、Ti/Te升高(P均<0. 05);与对照组比较,API阴性组治疗后TPTEF/TE、VPEF/VE、Ti/Te、VT/kg、TEF75降低(P均<0. 05);与对照组比较,API阳性组治疗后TPTEF/TE、VPEF/VE、Ti/Te、VT/kg、TEF25、TEF75降低(P均<0. 05)。结论潮气呼吸肺功能检测能反映婴幼儿喘息患儿肺功能改善情况,TPTEF/TE、VPEF/VE、Ti/Te可反映婴幼儿喘息患儿阻塞性通气功能障碍程度,VT/kg并非患儿通气功能改善的敏感指标。 Objective To investigate the dynamics of tidal breathing pulmonary function in infants with recurrent wheeze and its clinical significance. Methods A total of 114 infants with recurrent wheeze were enrolled and divided into the asthma predictive index( API) positive group( n = 50) and API negative group( n = 64),and the infants in both groups were given anti-inflammatory,anti-allergic,diastolic airway and other standard treatments. Meanwhile,another 50 healthy children were enrolled as the control group. Tidal breath pulmonary function at the time of admission( acute phase)and a week after discharge( remission phase) were tested,the ratio of time to peak tidal expiratory flow as a proportion of expiratory time( TPTEF/TE),volume to peak expiratory flow as a proportion of exhaled volume( VPEF/VE),tidal volume per kilogram( VT/kg),ratio of inspiratory time to expiratory time( Ti/Te),tidal expiratory flow at 75% lung volume( TEF75),tidal expiratory flow at 50% lung volume( TEF50),and tidal expiratory flow at 25% lung volume( TEF25)between groups were compared. Results TPTEF/TE,VPEF/VE,and Ti/Te increased in the API positive group and API negative group after treatment as compared with those of the same groups before treatment,with significant statistical difference( all P < 0. 05). Compared with the control group,TPTEF/TE,VPEF/VE,Ti/Te,VT/kg,and TEF75 decreased in the API negative group after treatment( all P < 0. 05). Compared with the control group,TPFEF/TE,VPEF/VE,Ti/Te,VT/kg,and TEF25,and TEF75 decreased in the API positive group after treatment( all P < 0. 05). Conclusion TPTEF/TE,VPEF/VE,and Ti/Te rather than VT/kg in the tidal breathing pulmonary function test can evaluate the control and severity of airway obstruction for infants with recurrent wheeze.
