摘要
目的:探究程序性死亡因子1(PD-1)/程序性死亡因子配体1(PD-L1)在复发性流产患者外周血T淋巴细胞中的表达及意义。方法:选取2015年7月至2017年2月本院收治的复发性流产患者57例为观察组,另选取42例正常妊娠妇女为对照组,采用流式细胞仪检测两组PD-1/PD-L1在外周血T淋巴细胞表达水平。并对患者实施淋巴细胞免疫治疗,检测治疗过程中PD-1/PD-L1水平变化情况及妊娠结局。结果:观察组CD3+、CD8+显著低于对照组,CD4+、CD4+/CD8+显著高于对照组(P<0.05);观察组PD-1/CD4+、PD-1/CD8+、PD-L1/CD4+、PD-L1/CD8+显著低于对照组(P<0.05);患者治疗后4周、治疗后8周PD-1/CD4+、PD-1/CD8+、PD-L1/CD4+、PD-L1/CD8+均显著高于治疗前(P<0.05);妊娠成功43例,妊娠失败14例,妊娠成功组PD-1/CD4+、PD-1/CD8+、PD-L1/CD4+、PD-L1/CD8+均显著高于治疗前,且显著高于妊娠失败组(P<0.05)。结论:PD-1/PD-L1在复发性流产患者外周血T淋巴细胞中呈低水平表达,可能参与疾病发生发展进程,影响妊娠结局,其可作为临床治疗潜在靶点。
Objective:To investigate the expression and significance of programmed death factor-1(PD-1)/programmed death factor-ligand 1(PD-L1)in peripheral blood T lymphocytes of patients with recurrent abortion.Methods:57 cases of recurrent spontaneous abortion treated in our hospital from July 2015 to February 2017 were selected as the observation group,another 42 normal pregnant women were selected as control group,the expression levels of PD-1/PD-L1 in peripheral blood T lymphocytes of two groups were detected by a loss cell analyzer.Lymphocyte immunotherapy was also applied to the patients,the change of PD-1/PD-L1 level during treatment and pregnancy outcome were detected.Results:The CD3+and CD8+in the observation group were significantly lower than those in the control group,CD4+and CD4+/CD8+were significantly higher than those in the control group(P<0.05);the PD-1/CD4+,PD-1/CD8+,PD-L1/CD4+and PD-L1/CD8+in the observation group were significantly lower than those in the control group(P<0.05);the levels of PD-1/CD4+,PD-1/CD8+,PD-L1/CD4+and PD-L1/CD8+at 4 weeks and 8 weeks were significantly higher after treatment(P<0.05);pregnancy was successful in 43 cases,and failed in 14 cases,PD-1/CD4+,PD-1/CD8+,PD-L1/CD4+and PD-L1/CD8+in successful pregnancy group were significantly higher than those before treatment,and significantly higher than the failed group(P<0.05).Conclusion:The expression of PD-1/PD-L1 in peripheral blood T lymphocytes of patients with recurrent spontaneous abortion is low,which may be involved in the occurrence and development of the disease and affect the outcome of pregnancy,it can be used as a potential target for clinical treatment.
