高频振荡通气治疗新生儿支气管肺发育不良的疗效观察 被引量:3

摘要 目的观察评价分析高频振荡通气(HFOV)治疗新生儿支气管肺发育不良(BPD)的临床效果。方法选择2017年6月至2019年2月期间惠东县人民医院儿科及惠州市中心人民医院儿科收治的新生儿支气管肺发育不良(BPD)患儿82例,随机对照试验(RCT)分为两组,对照组41例给予常规持续气道正压治疗;观察组41例给予高频振荡通气治疗,并比较两组的临床效果。结果观察组氧暴露时间与机械通气时间均显著地少于对照组(t=43.3027、21.8226,均P<0.05);两组BPD患儿治疗前、治疗后48 h的PaO2与PaCO2值在组内与组间比较差异均无统计学意义(均P>0.05);观察组与对照组治疗后48 h的OI值均显著地低于治疗前(t=6.5238、4.3047,均P<0.05);观察组治疗后48 h的OI值显著低于对照组(t=3.6652,P<0.05);观察组并发症发生率明显低于对照组[4.88%比29.27%],两组比较差异有统计学意义(χ^2=8.6134,P<0.05)。结论高频振荡通气(HFOV)治疗新生儿支气管肺发育不良(BPD)患儿能够显著地改善氧合指数,缩短氧暴露时间与机械通气时间,且有效地减少并发症的发生,值得临床大力推广应用。 Objective To observe and evaluate the clinical effects of high frequency oscillatory ventilation(HFOV)in the treatment of neonatal bronchopulmonary dysplasia(BPD).Methods From June 2017 to February 2019,82 children with neonatal bronchopulmonary dysplasia(BPD)admitted to the Department of Pediatrics of Huidong County People's Hospital and Huizhou Central People's Hospital were randomly divided into two groups.41 children in the control group received conventional continuous positive airway pressure therapy;41 children in the observation group were treated with high-frequency oscillatory ventilation.The clinical effects of the two groups were compared.Results The oxygen exposure time and mechanical ventilation time of the observation group were significantly lower than those of the control group(t=43.3027,21.8226,P<0.05).The PaO2 and Pa CO2 values of the two groups of BPD children before and 48 hours after treatment were not statistically significantly different(P>0.05).The OI values of the observation group and the control group at 48 hours after treatment were significantly lower than those before treatment(t=6.5238,4.3047,P<0.05);the OI value of the observation group at 48 hours after treatment was significantly lower than that of the control group(t=3.6652,P<0.05).The incidence of complications of the observation group was 4.88%,which was significantly lower than that of the control group(29.27%)(χ^2=8.6134,P<0.05).Conclusion High frequency oscillatory ventilation(HFOV)can significantly improve the oxygenation index for children with bronchopulmonary dysplasia(BPD),shorten the oxygen exposure time and mechanical ventilation time,and effectively reduce the occurrence of complications,which is worthy of clinical promotion and application.
