摘要
目的观察冠心病合并糖尿病患者应用双侧乳内动脉冠状动脉旁路移植术的治疗效果。方法选取本院81例冠心病合并糖尿病患者,依据患者糖化血红蛋白(HbA1c)水平进行分组,实验1组40例患者HbA1c水平>6.5%,实验2组41例患者HbA1c水平≤6.5%,另选取同期进行手术治疗的40例未合并糖尿病的冠心病患者作为对照组,对比三组患者手术效果及术后并发症发生率。结果3组患者LVEF水平、手术成功率、围术期死亡率及动脉闭塞率比较差异无统计学意义(P>0.05);对照组与实验2组切口并发症发生率差异无统计学意义(P>0.05),实验1组切口并发症发生率高于对照组及实验2组(22.50%比2.50%比4.88%)(P<0.05)。结论将合并糖尿病的冠心病患者的糖化血红蛋白水平控制在正常范围,并不会增加双侧乳内动脉冠状动脉旁路移植术并发症发生率。
Objective To observe the therapeutic effect of bilateral internal mammary artery coronary artery bypass grafting in patients with coronary heart disease and diabetes.Methods Eighty-one patients with coronary heart disease and diabetes mellitus were enrolled in our hospital.According to the patients'glycosylated hemoglobin(HbA1c)level,the HbA1c level of 40 patients in the experimental group 1 was>6.5%,and the HbA1c level of 41 patients in the experimental group 2 was≤6.5%.Forty patients with coronary heart disease without diabetes were selected as control group.The surgical outcome and postoperative complication rate of the three groups were compared.Results There were no statistically significant differences in LVEF level,surgical success rate,perioperative mortality,and arterial occlusion rate among the three groups(P>0.05).There was no statistically significant difference in the incidence of incision complications between the control group and the experimental group 2(P>0.05);the incidence of incision complications in the experimental group 1 was higher than that in the control group and the experimental group 2(22.50%vs.2.50%vs.4.88%)(P<0.05).Conclusion The glycosylated hemoglobin level in patients with diabetic coronary heart disease is controlled within the normal range and does not increase the incidence of complications of bilateral internal mammary artery bypass grafting.
