上海张江科学城院外心脏骤停病例分析及启示 被引量:11

Out of Hospital Cardiac Arrest in Shanghai Zhangjiang Science City:A Retrospective Analysis

摘要 目的通过回顾性分析张江科学城院外心脏骤停事件发生特点和急救现状,为健全该城区院外公众急救体系提供参考。方法对2012年1月一2017年7月在张江科学城地理规范范围内的558例院外心脏骤停(out-of-hospital cardiac arrest,OHCA)患者资料,采用Utstein模式进行转录,对其一般资料、心脏骤停资料、现场急救情况和患者结局情况进行统计分析。结果本组资料OHCA事件发生高峰是6:00-11:59时段(P<0.01),约73.30%的OHCA事件发生在居民区家中,心脏骤停的病因以心源性为主(368例,65.95%),被目击的OHCA事件仅14例(2.51%),急救响应时间(14.10±5.33)min,首次监测心律均为不可除颤心律,仅1例患者被实施了旁观者CPR,而旁观者除颤率为0,186例患者具有专业复苏措施记录,但无1例恢复自主循环。结论张江科学城OHCA院外公众急救现状不容乐观,今后可通过加大公众急救参与度,加强AED科学化配置,加快OHCA注册数据库建设,不断完善院外公众急救体系,进而改善该城区OHCA存活率和预后。 Objective Through a retrospective analysis of the characteristics and emergency status of the sudden cardiac arrest in Zhangjiang Science City,it is purposed to provide reference for the improvement of the public emergency system in the city.Method From January 2012 to July 2017,the data of 558 OHCA patients within the geographical specification of Zhangjiang Science City were transcribed with Utstein mode,and their general data,cardiac arrest data,on-site emergency treatment and patient outcome were statistically analyzed.Results The peak time of the OHCA events was from 6:00~11:59(PvO.Ol).About 73.30%of the OHCA cases occurred at home.The main cause of cardiac arrest is Cardiogenic(368,65.95%).Only 14 cases(2.51%)of OHCA events were witnessed,and the emergency response time was 14.10±5.33min.The first monitoring rhythm was non-defibrillation rhythm.Only one patient was performed bystander CPR,and the bystander defibrillation rate was zero.186 patients were recorded with professional resuscitation measures,but none was in returned of spontaneous circulation.Conclusions The current situation of OHCA pre-hospital public first aid in Zhangjiang Science City is not optimistic.In the future,we can improve the survival rate and prognosis of OHCA in the city by increasing the participation of public first aid,strengthening the scientific configuration of AED,accelerating the construction of OHCA registration database,in order to constantly improve the pre-hospital public first aid system.
