摘要
目的:探讨腰间盘突出症CT影像与临床症状严重程度的关系。方法:取2015年7月~2019年6月在本院治疗的116例腰椎间盘突出症患者为研究对象。所有患者均进行CT扫描,使用GE Prospeed FⅡ型CT机,电压120kV,电流100mA,扫描时间3s,扫描L3~4、L4~5、L5~S1部位,螺距3mm,层厚3mm。评估患者临床症状严重程度,并分析其与CT影像学的相关性。结果:116例患者轻度38例(32.76%),中度52例(44.83%),重度26例(22.41%)。从CT影像学表现上看,不同症状严重程度患者在髓核突出程度(矢状面)、经骨突出情况、许莫氏结节方面无明显差异(P>0.05);在髓核密度、髓核突出程度(额状位、水平面)、神经受压程度方面存在明显差异(P<0.05)。中度、重度患者的硬膜囊前后径明显大于轻度患者,矢状径小于轻度患者(P<0.05);而重度患者的硬膜囊前后径明显大于中度患者,矢状径小于中度患者(P<0.05)结论:分析CT影像表现及相关指标与腰间盘突出症患者病情严重程度存在相关性,对判断患者临床症状严重程度具有重要意义。
Objective: To investigate the relationship between CT images of lumbar disc herniation and the severity of clinical symptoms. Methods: From July 2015 to June 2019, 116 patients with lumbar disc herniation were treated in our hospital. All patients underwent CT scans using a GE Prospeed FII CT machine with a voltage of 120 kV, a current of 100 mA, a scan time of 3 s, and a scan of L3 to 4, L4 to 5, and L5 to S1, with a pitch of 3 mm and a layer thickness of 3 mm. The severity of clinical symptoms was assessed and analyzed for correlation with CT imaging. Results: 116 patients were mild in 38 cases(32.76%), moderate in 52 cases(44.83%), and severe in 26 cases(22.41%). From the CT imaging findings, patients with different severity of symptoms had no significant difference in the degree of nucleus pulposus(sagittal plane), trans-segmentation, and Xu Mo’s nodules(P>0.05);There were significant differences in the degree of nuclear protrusion(frontal position, horizontal plane) and nerve compression(P<0.05). The anteroposterior diameter of the dural sac was significantly greater in patients with moderate or severe disease than in mild patients(the sagittal diameter was smaller than that in mild patients)(P<0.05). The anteroposterior diameter of the dural sac was significantly greater in patients with severe disease than in moderate patients. Patients with degree(P<0.05) concluded that CT imaging findings and related indicators were correlated with the severity of patients with lumbar disc herniation, which is important for judging the severity of clinical symptoms.
